正清风痛宁联合郑氏伤科推拿治疗慢性创伤性膝关节滑膜炎  

A Clinical Observation on 46 Cases of Chronic Traumatic Knee Synovitis Treated with Zhenqingfengtongnin Transdermal Administration plus Zheng's Massage

摘　　要：目的:观察正清风痛宁透皮给药联合郑氏伤科推拿治疗慢性创伤性膝关节滑膜炎的疗效。方法:将46例确诊的慢性创伤性膝关节滑膜炎患者随机分为对照组、治疗组各23例:分别采用依托芬那酯凝胶外涂法和正清风痛宁透皮给药联合郑氏伤科推拿治疗。治疗前、治疗后2周、4周采用疼痛视觉类比评分(VAS)、Bristol膝关节评分系统进行评定。并于治疗后3、6个月电话随访两组患者的自觉复发情况及监测不良反应发生情况。结果:(1)与治疗前比较,治疗后两组Bristol膝关节评分系统、VAS评分差异有统计学意义(P<0.05);与治疗后2周比较,疗后4周治疗组Bristol膝关节评分系统、VAS评分差异有统计学意义(P<0.05),对照组差异无统计学意义(P>0.05);与对照组比较,治疗组Bristol膝关节评分系统、VAS评分差异有统计学意义(P<0.05)。(2)治疗结束后3个月随访,治疗组有1例复发,对照组有4例复发;治疗结束后6个月随访,治疗组有2例复发,对照组有8例复发。(3)治疗组未见明显不良反应,对照组3例出现皮肤过敏反应。结论:正清风痛宁透皮给药联合郑氏伤科推拿治疗慢性创伤性膝关节滑膜炎疗效显著,复发率低,安全性高,是一种值得临床推广的治疗方案。Objective：To observe the clinical＇s massage. Methods： According to random curative effect of Zhengqingfengtongning number table, forty-six patients met the transdermal administration plus Zheng criteria were randomly divided into 2 groups,23 patients in each group. Patients in the control group were administrated with the Etofenamate gel external application, while the treatment group was treated with Zhengqing fengtongning transdermal administration plus Zheng＇s massage. 2 and 4 weeks after treatment, the curative effects were evaluated according to the visual analogue scale and Bristol knee score system. Recurrence and adverse reaction were followed up through telephone at 3 and 6 months after the treatment. Results： （1）In these 2 groups, there was significant difference of Bristol knee score and VAS score pre-and post-treatment （P〈0. 05）. In the treatment group, the Bristol knee score and VAS score 2 weeks after treatment were significant different with those 4 weeks after treatment （P〈0.05）, while there was no difference in the control group （P〉0.05）. There was significant difference of Bristol knee score and VAS score between the control group and the treatment group. （2）At the end of 3 months of follow-up, there were 1 cases recurred in the treatment group and 4 cases in the control group. At the end of 6 months of follow-up, there were 1 cases recurred in the treatment group and 4 cases in the control group. （3）The treatment group showed no obvious adverse reaction and 3 cases have skin allergy in the control group. Conclusion：Zhengqingfengtongning transdermal administration combined with Zheng＇s massage shows obvious curative effect, low recurrence rate and high safety. It＇s worthy of clinic application.
