应用两种超声龈上洁治器械治疗菌斑性龈炎的临床疗效及患者主观感受评价

摘　　要：目的:观察两种超声龈上洁治器械治疗菌斑性龈炎的临床疗效及釉质光洁度,评价不同年龄群的患者在治疗过程中的主观感受。方法:采用自体对照,随机单盲设计,通过牙龈出血指数(GBI)及探诊出血(BOP)阳性位点率评价临床疗效,使用视觉模拟标尺(VAS)评价患者主观感受,并按年龄分为两组,评价老年人群及中青年人群对洁治的不同反应。通过扫描电镜观察操作后釉质表面情况。结果:老年人群使用磁致伸缩式洁治时(M组)患者的VAS分值平均为30.07,低于使用压电陶瓷式(P组,41.25),中青年人群的VAS均值分别为29.15(M组)及39.10(P组),均有显著性差异。不同人群使用同一种处理方式其VAS均值之间无显著性差异。一个月后两种人群的GBI及BOP阳性位点率均比治疗前有相应明显降低,但两种处理方式之间无明显差异。扫描电镜结果显示M组对牙釉质表面损伤较小,釉面更为平整。结论:应用两种洁治器均能有效治疗菌斑性龈炎且疗效类似,使用磁致伸缩式洁治器有降低患者不适及减少釉质损伤的优点。Objective： To investigate the clinical outcomes of gingivitis treated with two types of ultrasonic scalers,and to evaluate the pain perception of patients and the smoothness of the enamel surface.Methods： The study was designed as self-control and random single blind one.The gingival bleeding index（GBI） and positive sites rates of bleeding on probe（BOP） was used to evaluate the clinical outcomes of treatment.The pain perception of patients was investigated by visual analog scale（VAS） score.The study was investigated in different samples based on ages respectively.The enamel surface was observed by scanning electron microscope.Results： In the group above 65 yrs,the mean VAS score of pain was 30.07 when using magnetostrictive scaler（M group）,which was significant different compared with using piezoelectric scaler（P group,41.25）.The similar outcomes were observed in the group under 65 yrs.The GBI and the positive sites rates of BOP decreased in both M and P group 1 month later.There was no significant difference between both groups when comparing the GBI and positive sites rates of BOP.However,the enamel surface of M group seemed smoother and showed less damages than P group.Conclusion： There were similar clinical outcomes of treating gingivitis between both types of scalers.But the magnetostrictive one seemed to result in less pain and less enamel damage for patients.
