欣母沛在预防高危产妇产后出血中的价值及使用时机探讨

摘　　要：目的:探讨欣母沛在预防高危产妇产后出血的价值及应用时机。方法:将122例存在产后出血高危因素孕妇随机均分为欣母沛A组与欣母沛B组,选择未使用欣母沛药物高危孕妇61例编入常规组.欣母沛A组在产后出血且止血无效时使用欣母沛.欣母沛B组在胎儿娩出后立即使用欣母沛.对比三组患者间产后2 h,24 h出血量及产后出血率。结果:常规组在术中出血量、产后2 h出血量及产后24 h出血量均明显高于欣母沛A组与欣母沛B组(p<0.01);欣母沛A组在术中出血量、产后2h出血量及产后24 h出血量均明显高于欣母沛B组(P<0.01)。欣母沛B组产后出血率低于欣母沛A组(P<0.05).结论:欣母沛相比缩宫素具有更好防治产后出血效果。对于具有产后出血高危因素产妇,应在胎儿娩出后立即使用欣母沛.Objective： To discuss the effect and when to use of hemabate in the treatment of preventing Postpartum hemorrhage.Methods： Randomly divided 122 cases of puerperas who had postpartum hemorrhage risk factors into the hemabate A group and hemabate B group.Choosing 61 cases of puerperas who had postpartum hemorrhage risk factors but no using hemabate into the normal group.In the hemabate A group,hemabate was used when the postpartum hemorrhage can＇t be controlled by normal drug.And the hemabate B group used hemabate immediately when the delivery of baby.Compared the intraoperatve blood soss,the volume of hemorrhage 2 hours and 24hours after parturition and the postpartum hemorrhage rate among the three groups.Results： The normal group had higher result than the hemabate group in the intraoperatve blood soss,the volume of hemorrhage 2 hours and 24 hours after parturition（P0.01）.In the hemabate A group,the intraoperatve blood soss,the volume of hemorrhage 2 hours and 24 hours after parturition（P0.01） than those in the hemabate B group.The hemabate B group had lower postpartum hemorrhage rate than the hemabate A group（P0.05）.Conclusion： Hemabate has a better effect in the preventing of postpartum hemorrhage than the Pitocin.It should be used immediately after the delivery of baby for the puerperas who had postpartum hemorrhage risk factors.
