摘 要:目的探讨超选择性冠状动脉内人脐带血单个核细胞移植治疗冠心病的可行性及安全性。方法选择经冠状动脉造影明确诊断为多支血管弥漫病变的冠心病患者15例,桡动脉穿刺建鞘,通过指引导管将非快速交换球囊(over-the-wier)超选择到左前降支,4~6atm充盈球囊,经球囊中央孔将5mL脐带血单个核细胞缓慢灌注至前降支。全程监测心电、血压、血氧饱和度,于术前、术后即刻、术后1周取外周静脉血检测肝肾功能、肌钙蛋白T(TnT)水平变化,并记录不良反应及心血管事件发生情况。结果 (1)所有患者均成功完成手术治疗,平均手术时间(12.6±3.2)min,无一例发生不良反应及心血管事件;(2)与术前比较,术后肝肾功能及TnT水平差异无显著性(P>0.05)。结论超选择性冠状动脉内脐带血单个核细胞移植治疗冠心病是一种安全可行的方法。Objective To investigate the probability and safety of intra-coronary transplantation of human cord blood mononuclear cell for the treatment of coronary heart disease patients. Methods 15 coronary heart disease patient with multi vessel disease were chosen, human cord blood mononuclear cells were infused into left anterior descending branch by percutaneous transluminal catheter technique through radial artery. 4-6atm inflated the balloon and 5ml human cord blood mononuclear cells were injected through the central cavity of the over-the-wire balloon. Blood pressure, electrocardiogram and blood oxygen saturation were recorded throughout the operation. Changes of CK(cardiac kinase), CK- MB(cardiac kinase-MB), acetic transaminase (AST), alanine transaminase (ALT), lactic dehydrogenase (LDH), creatinine (Cre), blood urea nitrogen (BUN) and cardio troponin T(TnT) were detected before and after the procedure. Other side effects and cardiac events were recorded. Results All patients finished the operation successfully, and the mean operation time was (12.6 ±3.2) minutes. No side effects or cardiac events occurred. There's no changes on liver and kidney function and there were no statistically significant differences the serum TnT level before and after the human cord mononuclear cells transplation (P〈0.05). Conclusion Intra-coronary transplantation of human cord mononuclear cells to deal with coronary heart disease patient might be a safety and possible approach.
