乙酰半胱氨酸泡腾片治疗慢性阻塞性肺疾病急性加重期的疗效观察

摘　　要：目的:观察乙酰半胱氨酸泡腾片(N-acetylcysteine effervescent tablets,NAC)在治疗慢性阻塞性肺疾病急性加重期(acuteexacerbation of chronic obstructive pulmonary disease,AECOPD)的疗效。方法:以随机抽样法将符合诊断的AECOPD患者100例分为治疗组和对照组,每组各50例。2组患者在急性发作期均给以常规抗感染、解痉、吸入激素、吸氧、体位排痰、营养支持治疗;治疗组患者在常规治疗基础上加用乙酰半胱氨酸泡腾片600 mg,2次/d,治疗7 d;2组患者均不使用维生素C、维生素E及盐酸氨溴索和盐酸溴环己胺醇等抗氧化剂。观察所有入选病例的临床症状、血气分析、血常规等指标改善情况。结果:治疗组在临床疗效、症状体征改善及实验室检查指标改善方面均优于对照组(P<0.05),且不良反应发生率低。结论:AECOPD患者在常规治疗基础上加用乙酰半胱氨酸泡腾片泡服可显著提高疗效。OBJECTIVE：To observe the curative efficacy of N-acetylcysteine effervescent tablets for patients with acute exacerbation of chronic obstructive pulmonary disease（AE-COPD）. METHODS： 100 patients meeting the diagnosis criteria of AE-COPD were randomly assigned to either treatment group or control group of 50 each. Both groups were conventionally given anti-infective and antispasmodic medication plus inhalation of corticosteroids and oxygen, postural expectoration, and nutritional support yet without administration of vitamin C, vitamin E and antioxidants such as ambroxol hydrochloride etc. However,the treatment group received add-on N-acetylcysteine effervescent tablets at a dosage of 600 rag,2 times/d for 7 days. The improvement in clinical symptoms,blood gas analysis,blood routine and other indicators were followed. RESULTS： The treatment group showed better improvement in clinical efficacy, signs and symptoms and laboratory parameters than did the control group （P 〈 0. 05）, and the treatment had lower incidence of adverse reactions than did the control group. CONCLUSION： Treatment of AE-COPD patients with add-on N-acetylcysteine effervescent tablets resulted in s^gnificant^y better curative efficacy than did with conventional therapy alone.
