曲安奈德鼻喷雾剂对变应性鼻炎患者血清白介素-4和γ-干扰素水平的影响及疗效观察

摘　　要：目的观察曲安奈德鼻喷雾剂对变应性鼻炎患者血清白介素(IL)-4和γ-干扰素(IFN-γ)水平的影响及疗效观察。方法 72例变应性鼻炎患者随机分为观察组和对照组。两组均予以口服氯雷他定片10 mg,1次/d,共4周,观察组加用曲安奈德鼻喷雾剂喷鼻,2次/d,连用4周。观察两组患者治疗前后血清IL-4和IFN-γ水平的变化,并进行临床疗效及安全性的观察。结果治疗4周后,两组患者血浆IL-4水平均较前明显下降,IFN-γ水平均较前明显上升(P<0.05),且观察组下降或上升幅度明显大于对照组(P<0.05);同时观察组临床总有效率明显优于对照组(χ2=5.26,P<0.05)。两组药物不良反应发生率比较无明显统计学差异(χ2=0.46,P>0.05),未发生严重药物不良反应。结论曲安奈德鼻喷雾剂治疗变应性鼻炎疗效确切、安全性较好,作用与其能降低血清IL-4水平、提高血清IFN-γ水平密切相关。Objective To discuss the influence and curative observation of triamcinolone acetonide nasal spray on serum interleukin-4（IL-4） and interferon-γ（IFN-γ） levels of allergic rhinitis patients.Methods Seventy-two cases of allergic rhinitis patients were divided into observation group and control group at random.The patients in two groups were given loratadine tablets（10mg） one time daily for 4 weeks,while the patients in control group were given triamcinolone acetonide nasal spray two times daily for 4 weeks additionally.The changes of serum IL-4 and IFNγ levels of patients in two groups were observed before and after medical treatment,and the clinical curative effect and security observation were carried out.Results After 4 weeks＇ medical treatment,the IL-4 level in plasma of patients in two groups declined obviously than before,while IFN-γ level rose obviously than before（P 0.05）,and the declining or rising rate of observation group was much higher than that of control group（P 0.05）;Meanwhile,the total clinical efficiency in observation group was much better than that in control group（χ2=5.26,P 0.05）.After comparing the occurrence of drug adverse reaction（DAR） in two groups,no obvious statistical difference or serious DAR appeared in the medical treatment period（χ2=0.46,P 0.05）.Conclusion Triamcinolone acetonide nasal spray has reliable curative effect on allergic rhinitis with high security,and mechanism of action has close effect on reducing serum IL-4 level and enhancing serum IFN-γ level.
