摘　　要：目的探讨p57Kip2和Ki-67蛋白在妊娠早期鉴别诊断水肿性流产和完全性葡萄胎、部分性葡萄胎的应用价值。方法免疫组织化学染色法检测早期妊娠的12例水肿性流产、22例完全性葡萄胎和16例部分性葡萄胎石蜡组织切片中p57Kip2蛋白和Ki-67蛋白的表达。结果水肿性流产、完全性葡萄胎和部分性葡萄胎组织中,p57Kip2蛋白的阳性表达率分别为83.3%(10/12)、0(0/22)、81.3%(13/16),完全性葡萄胎组阳性表达率与部分性葡萄胎组和水肿性流产组比较,组间差异有显著统计学意义(P<0.01),部分性葡萄胎组与水肿性流产组比较,差异无统计学意义(P>0.05);Ki-67蛋白的阳性表达率分别为8.3%(1/12)、72.7%(16/22)、43.8%(7/16),完全性葡萄胎组阳性表达率与部分性葡萄胎组和水肿性流产组比较,组间差异有显著统计学意义(P<0.01),部分性葡萄胎组与水肿性流产组比较,差异有统计学意义(P<0.05)。结论联合检测p57Kip2和Ki-67蛋白对鉴别诊断早期妊娠中水肿性流产、完全性葡萄胎、部分性葡萄胎有重要临床应用价值。Objective To investigation the utility of p57Kip2 and Ki-67 proteins in differential diagnosis of hydropic abortion（HA）,complete hydatidiform mole（CHM） and partial hydatidiform mole（PHM） in early pregnancy stage.Methods Immunohistochemistry Envision method was adopted to detect the expression of p57Kip2 and Ki-67 proteins in paraffin-embedded tissue sections of 12 cases HA,22 cases CHM and 16 cases PHM.Results Positive staining rates of p57Kip2protein was 83.3%（10/12）,0（0/22）,81.3%（13/16） in 12 cases HA,22 cases CHM and 16 cases PHM,respectively.The difference of p57Kip2 protein expression between CHM and PHM or HA was statistically significant（P0.01）,but it was not statistically significant between PHM and HA（P0.05）.Positive staining rates of Ki-67 protein was 8.3%（1/12）,72.7%（16/22）,43.8%（7/16） in 12 cases HA,22 cases CHM and 16 cases PHM,respectively.The difference of Ki-67 protein expression between CHM and PHM or HA was statistically significant（P0.01）.The difference of Ki-67 protein expression between PHM and HA was statistically significant（P0.05）.Conclusion Codetecting p57Kip2 and Ki-67 proteins has significant clinical utility in differential diagnosis of CHM from PHM and HA,or PHM from HA in early pregnancy stage.
