2012年四川省攀枝花市不明原因猝死流行病学调查分析

摘　　要：目的查找2012年四川省攀枝花市不明原因猝死病例的发病原因。方法采取流行病学调查方法获取死者生前的各种暴露因素,实验室检测排除已知的可能病因。结果此起疫情在72小时内导致5例不明原因猝死,1例同发病例,均为彝族,流行病学线索追踪调查5例死者、1例同发病例,均有"小白菌"食用史,未食用者未发病,食用的10人中有6人发病;实验室检测排除鼠疫及肉毒毒素、常见化学毒物、常见农药中毒;病理切片提示3例死者为心源性猝死,第五例死者系窒息死亡。结论第五例死者经病理切片排除了不明原因猝死;从食用"小白菌"至发病的潜伏期、发病者共同的食用史、家族食用习惯,推测"小白菌"是发生此起不明原因猝死的高危因子。Objective To investigate onset reason for unexplained sudden death in Panzhihua City in 2012. Methods The field epidemiology survey was taken to get various exposure factors of the deceased cases, and laboratory testing was per- formed to rule out possible causes of the known. Results This outbreak led to five deaths and one homologous case within 72 hours. These cases were of Yi nationality and had the history of eating ＂white fungus＂ in certain family. The fifth death ate less. Villagers had the habits of eating wild mushroom with the family as a unit relatively fixed and generation to generation. The certain family had the heritage habit of eating ＂white fungus＂. Laboratory testing excluded plague and botulinum toxin, a common chemical poisons and common pesticide poisoning. Pathological section prompted that 3 deaths were sudden cardiac death and the fifth death died of asphyxiation. Conelusions The biopsy of the fifth dead case excludes sudden unexplained death. We speculate that ＂white fungus＂ is a high-risk factor of this unexplained sudden death on the basis of the period from eating ＂white fungus＂ to the incubation period of the disease, the incidence of common edible history and family habit of eat- ing white fungus.
