成人胆总管囊肿的诊治分析

摘　　要：目的总结成人胆总管囊肿的诊断和治疗经验。方法回顾性分析2000年～2012年收治的17例成人胆总管囊肿患者的临床资料，术后随访7个月-5年，平均3年。结果所有17例患者中行胆总管囊肿切除加肝管空肠Roux．Y吻合术14例，囊肿切除、胆总管修补、T管引流术2例，肝左外叶切除1例。术后伤口感染、腹腔出血各1例，经保守治疗后均好转，17例均治愈出院。结论胆总管囊肿切除加肝管空肠Roux．Y吻合术是手术治疗成人胆总管囊肿的首选术式，操作简单，术后并发症少。Objective To summarize the methods and experience for diagnosis and treatment for adult choledochal cyst.Methods The clinical data from 17 patients treated from 2000 to 2012 were analyzed retrospectively, and the time of following-up was 7 months to 5 years（average 36 months）.Results Operations included excision of the cyst with Roux-en-Y hepaticojejunostomy in 14 cases,excision of the cyst with drainage of biliary tract by type T catheter in 2 cases,left lateral lobe hepatectomy in 1 case,incision infection occurred in 1 case and intraperitoneal hemorrhage in 1 case.All complications were cured by conservative treatment.All 17 cases were cured and lest hospital.Conclusion Excision of the cyst with Roux-en-Y hepaticojejunostomy is simple,less postoperative complications,recommended as the therapy of choice in selective operation.
