64排CT在进展期胃癌和胃原发性淋巴瘤鉴别诊断中的应用价值

摘　　要：目的评价CT在进展期胃癌和胃原发性淋巴瘤鉴别诊断的应用价值。方法回顾性比较经病理证实的22例进展期胃癌和19例胃淋巴瘤的CT表现，其中包括病灶的部位、大小、增强程度、淋巴结转移以及其它一些征象。结果进展期胃癌病灶平均厚度为1．9cm，CT值在动脉期及门脉期分别增加20Hu和40HU；胃淋巴瘤病灶平均厚度为3．2cm，CT值在动脉期及门脉期分别增加10HU和20HU。在22例进展期胃癌患者，11例伴胃周淋巴结肿大，5例伴腹部转移且其中l例为胰腺侵犯；在19例胃淋巴瘤患者，5例为除胃以外的十二指肠、小肠、结肠等多部位病变，11例伴多区域淋巴结肿大，且其中8例伴腹膜后肾门下淋巴结肿大。结论CT检查可为胃淋巴瘤和进展期胃癌的鉴别诊断提供重要的信息。Objective To evaluate the value of CT in differential diagnosis between advanced gastric carcinoma and primary gastric lymphoma.Methods Retrospective comparison of CT images of surgically and pathologically confirmed 22 cases of advanced gastric cancer and 19 cases of primary gastric lymphoma patients,the radiological signs evaluated were observed for the site of lesion,thickness of lesion,degree of enhancement and enhancement pattern,invasion of other abdominal organs and abdominal lymph nodes,and so on.Results In carcinoma patients,the average thickness of the lesion is 1.9cm,increase in mean CT values in the arterial and portal venous phases were 20HU and 40HU respectively;in primary gastric lymphoma patients,the average thickness of the lesion is 3.2cm,increase in mean CT values were 10HU and 20HU.Of 22 cancer patients,perigastric lymphadenopathy in 11,abdominal metastasis in 5 with pancreatic invasion in 1 patient.Of 19 NHL cases,multiple sites lymphadenopathy in 11,retroperitoneal lymphadenopathy below renal hila in 8 were assessed and 5 showed multiple sites involvement other than stomach.Conclusion CT examination provides important information in the differential diagnosis between primary gastric lymphoma and advanced gastric carcinoma.
