摘 要:目的探讨脊椎骨嗜酸性肉芽肿的MRI表现。方法回顾性分析11例经病理证实的脊椎骨嗜酸性肉芽肿的MRI表现。结果发生于颈椎3例,胸椎4例,腰椎4例;单发8例,多发3例;病变椎体表现为楔形或铜钱样变扁,TIWI呈等或低信号,T2WI及STIR序列为高或混杂信号;可伴椎旁软组织肿块形成,邻近椎间盘形态正常或轻度膨降,破坏区内可见残存小骨片而无死骨存在;增强扫描,病变及周围软组织肿块呈明显强化。结论脊椎骨嗜酸性肉芽肿的MRI表现有一定特点,对该病的诊断具有重要价值。Objective To explore the MRI features of eosinophilic granuloma of spinal vertebra.Methods A retrospective study was conducted to evaluate MRI findings of 11 cases with vertebral eosinophilic granuloma. Results The location of vertebral eosinophilic granuloma lesions were in cervical vertebra(n=3),thoracic vertebra(n=4),lumbar vertebra(n=4),8 cases were solitary lesions,while 3 cases were multiple.The main MRI features of vertebral eosinophilic granuloma were wedge-shaped or copper-coin-shaped vertebrae can be identified in the series,equal or hypotensity signal on T1 weighted image,but hyper or mixed signal in T2 weighted image and STIR image.Besides,paravertebral soft mass can be detected and all cases had a normal or slightly bulged adjacent disc as well as remnants of small bony chip.However, no sign of sequestrum was identified.In addition,on enhanced images,significantly heterogeneous enhancing was noted in lesion and soft mass.Conclusion There exists specific MRI features,which may be of significant value in diagnosis of vertebral eosinophilic granuloma.
