血清降钙素原在ICU血流感染患者中的诊断价值分析

摘　　要：[目的]探讨血清降钙素原(procalcitonin,PCT)在重症监护室(ICU)感染患者诊断中的价值。[方法]采用电化学发光法对我院ICU 730例疑似感染患者进行血清PCT测定,同时采用BacT/Alert进行血培养分析和VETEKII细菌鉴定,对PCT结果和血培养结果之间进行比较分析。[结果]730例疑似感染患者血培养结果有55例阳性,其中34例革兰阳性(G+)球菌,18例革兰阴性(G-)杆菌,3例真菌。血培养阴性患者PCT为(0.14±0.09)ng.mL-1,血培养阳性患者PCT为(1.57±0.94)ng.mL-1,差异有统计学意义(P<0.05)。血培养阳性其中G+球菌患者PCT为(0.97±0.38)ng.mL-1,G-杆菌患者PCT为(4.21±1.73)ng.mL-1,差异有统计学意义(P<0.05)。[结论]血清降钙素原测定是诊断菌血症的一个有效补充,并有助于提高脓毒血症诊断的准确性,为临床抗生素的早期应用提供依据。[Objective]To explore the clinical value of serum procaltonin and blood culture in ICU patients with infection. [Method]Peripheral blood sam- ples from 730 suspected infection was comparative analysed, the semm procalcitonin was tested by using electrochemical luminescence method and the blood culture was tested by using BacT/Alert analysis and VETEKII identification of bacteria. [Results]There were 55 positive cases from 730 suspected in- fection including 34 cases of gram positive（G＋）, 18 cases of gram-negative（G-）, 3 cases of fungus.The PCT of blood culture negative was 0.14__.0.09 ng＂ mL-1 and the positive was 1.57_＋0.94 ng＇mL-1, the difference was statistically significant.The PCT of G＋ of blood culture positive was 0.97_＋0.38 ng＇mL-1 and the G- was 4.21＋1.73 ng mL-1, the difference was statistically significant. [Conclusion] Serum PCT is an effective supplement to the diagnosis ofbac- teremia and helps to improve the accuracy of diagnosis of sepsis, provides the help for the early clinical antibiotic application.
