水痘并脓毒性休克、多器官功能衰竭和弥漫性血管内凝血患者药物治疗方法

摘　　要：目的分析长期应用糖皮质激素者感染水痘的临床特点,探讨重症水痘的治疗和预防。方法通过回顾国内外关于重症水痘及其并发症的文献资料,对此例患者的临床表现和主要治疗药物进行分析。结果糖皮质激素导致患者免疫功能低下,易罹患致死性重症水痘,需及早有效抗病毒治疗,同时给予激素治疗,丙种球蛋白联合治疗的临床获益有待进一步证实。一定时期预防性接种水痘疫苗可以降低发生水痘及并发症的风险。结论长期应用糖皮质激素者,应当警惕重症水痘的发生,尽早积极抗病毒并给予激素治疗,预防性接种水痘疫苗可能使部分人群获益。Objective To analyse the clinical characteristics of varicella in patients with long-term application of glucocorticoids and investigate the treatment and prevention of severe varicella. Methods By reviewing the literature at home and abroad about sever varicella and its complications,the clinical manifestations and main medicines of this case is analyzed. Results Individuals with underlying glucocorticoids-induced immunocompromise are at high risk of developing sever,life-threatening varicella,positive antiviral therapy with glucocorticoid should be given as soon as possible. But more work should be done to confirm the clinical benefit of combining immunoglobulin. Preventive vaccination of varicella at certain period of time can reduce the risk of varicella and its complications. Conclusion Patients with long-term application of glucocorticoids should be alert to the occurrence of intensive varicella,applying active antiviral therapy with glucocorticoid is necessary, and some of them may benefit from preventive inoculation of varicella vaccine.
