两种宫颈准备方法用于绝经后宫内节育器取出

摘　　要：目的对比戊酸雌二醇+米索前列醇和宫术宁宫颈扩张棒分别用于绝经后女性宫内节育器取出。方法选择首次常规取器因宫颈不能扩张,拟再次取器的绝经后女性153例,分为A组86例,B组67例,A组使用戊酸雌二醇(补佳乐)+米索前列醇序贯应用的方法,B组使用宫术宁宫颈扩张棒(以下简称宫术宁胶棒)进行宫颈准备后常规取器或B超引导下取器。观察两组的宫颈准备效果、术前术中不良反应发生率、手术时间以及取器效果。结果两组宫颈准备效果、术中不良反应发生率、手术时间以及取器效果相似,差异无统计学意义(P>0.05)。术前宫颈准备时的不良反应发生率B组明显低于A组,两组相比,差异有统计学意义(P<0.05)。结论两种宫颈准备的方法用于绝经后宫内节育器取出时,宫颈扩张效果俱佳,宫术宁胶棒使用方便、简单、无不良反应,特别适用于患有禁忌使用雌激素和米索前列醇的合并症的受术者进行宫颈准备。Objective To discuss the clinical value of using estradiol valerate tablets and misoprostol tablets in combination and gongshuning cervical dilator respectively for cervical preparation in postmenopausal women with IUD removal difficulties,and to seek a cervical preparation method suitable.Methods 153 postmenopausal,again,were divided into group A（86 cases） and group B（67 cases）.Women in group A were given estradiol valerate tablets and Misoprostol tablets in sequential administration and those in group B were placed with Gongshuning cervical dilator（hereinafter referred to as Gongshuning rod for short） for cervical preparation before routine IUD removal or B-mode ultrasound induced IUD removal.Then the effect of cervical preparation,the incidence rate of preoperative and intraoperative adverse reactions,the operation time and the effect of IUD removal were observed.Results The effect of cervical preparation,the incidence rate of intraoperative adverse reactions,the operation time and the effect of IUD removal were similar in both groups（P0.05）;the incidence rate of adverse reactions in group B during preoperative cervical preparation was significantly lower than that in group A（P0.05）.Conclusion Both cervical reparation methods used in postmenopausal women with IUD removal difficulties were effect;Gongshuning rod was easy and simple to use,free of side effect,and especially suitable for use in cervical preparation among women with complications where estradiol valerate tablets and misoprostol tablets were contraindicated.
