摘  要:目的:评价前门控128层螺旋CT血管造影(CTA)诊断婴儿肺静脉畸形引流的应用价值。方法:肺静脉畸形引流患儿12例,采用128层螺旋CT前门控扫描,CTA图像后处理包括多平面重组(MPR)、容积重建(VR)、最大密度投影(MIP)及最小密度投影(MinP)。评价肺静脉畸形引流类型、气道异常及并发心内畸形。结果:完全型肺静脉畸形引流9例,包括心上型5例、心内型3例和心下型1例;部分型肺静脉畸形引流3例,包括心上型2例和心内型1例。并发房间隔缺损12例、动脉导管未闭2例、气道畸形3例。术前CTA诊断与手术结果完全相符。结论:前门控128层螺旋CTA在诊断婴儿肺静脉畸形引流中具有重要价值,能够为心外科手术方案制定提供可靠依据。Purpose: To evaluate the value of prospectively ECG-gated 128-slice spiral CTA in the diagnosis of anomalous pulmonary venous drainage (APVD). Methods: Twelve paediatric patients of APVD were undergone by prospectively ECG-gated 128-slice spiral CTA. The APVD types, airway abnormality and acompanied intracardiac malformation were evaluated by the multiplanar reconstruction (MPR), volume rendering (VR), maximum intensity projection (MIP) and minimum intensity projection (MinP). Results: Anomalous pulmonary venous drainage were shown in 9 cases, 5 cases of them were central type, 3 cases were intracardiac type, 1 case was infracardiac type; partial anomalous pulmonary venous drainage were shown in 3 cases, 2 cases of them were central type, 1 cases was intracardiac type. Twelve cases were accopanied with atrial septal defect, 2 cases with patent ductus arteriosus, and airway malformation was shown in 3 cases. The preoperative CTA diagnosis and the operation result was fully consistent. Conclusion: 128-slice spiral CTA with gating technique has important value in the preoperative diagnosis of pediatric pulmonary venous anomalous.
