摘 要:目的分析宫内节育器(intrauterine device,IUD)避孕大鼠模型无创法(经阴道—宫颈途径)与开腹法建模腹腔冲洗液中肿瘤坏死因子-α(TNF-α)、转化生长因子-β(TGF-β)、细胞间粘附分子-1(ICAM-1)的变化,初步评价成模效果。方法选择已生育SD雌性大鼠50只,随机分为无创组、开腹组、无创假手术组、开腹假手术组、空白对照组,每组10只;无创组、开腹组分别采用无创法(经阴道—宫颈)和开腹法(经腹腔—子宫)置入单侧宫腔含铜IUD,假手术组置入IUD后立即取出,造模后第16天,检测各组大鼠腹腔冲洗液TNF-α、TGF-β、ICAM-1浓度,并观察子宫内膜病理学改变。结果与空白对照组比较,无创组、开腹组、开腹假手术组TNF-α浓度均明显升高(所有P<0.05),开腹组TNF-α浓度升高最明显,达到极显著水平(P<0.01),亦显著高于无创组TNF-α浓度(P<0.01)。与空白对照组比较,开腹组、开腹假手术组TGF-β浓度均明显升高(P<0.05),开腹组TGF-β浓度升高最明显,达到极显著水平(P<0.01),亦显著高于无创组TGF-β浓度(P<0.01)。各组ICAM-1浓度,组间差异则无显著性(P>0.05)。无创组、开腹组子宫内膜均存在上皮缺损,炎细胞浸润、充血水肿伴增生,血管腔扩大并散在血栓,开腹组IUD置入侧的病理损伤更为明显。结论模拟临床IUD放置的无创式造模法,较之传统的开腹法,能够明显降低IUD避孕大鼠模型非造模目的的腹腔干扰因素刺激,可能更能反映临床IUD避孕过程。Objective To analyze the levels of TNF-a, TGF-β and ICAM-1 in peritoneal lavage fluid of IUD contraceptive rat models established by non-invasive modeling and laparotomy modeling. Methods Fifty female SD muhiparous rats were randomly assigned into non-invasive, laparotomy, non-invasive control, laparotomy control, and blank control groups, 10 rat in each group. Copper-intrauterine device (IUD) was placed into the unilateral uterine of rat in the noninvasive group through vagina-cervix (non-invasive) way and that of invasive group via laparotomy. The rats were also operated in the non-invasive control and laparotomy control groups but without IUD placing. The concentrations of TNF- or, TGF-13, and ICAM-I in the peritoneal lavage fluid and pathological change of endometrium were examined at 16 days after modeling. Results The TNF-ct of non-invasive group, laparotomy group, and laparotomy sham operation group were significantly higher than that in the blank control group (P 〈 0. 05). It was highest in the laparotomy group, and was significantly higher than that of the non-invasive group (P 〈 0. O1 ). Compared with the non-invasive control group, the level of TNF-ct was significantly higher in the laparotomy group and laparotomy sham operation group (P 〈 0. 01 or P 〈 0.05). It was highest in the laparotomy group, and was significantly higher than that of the non-invasive group (P 〈 0.01 ). The concentrations of ICAM-1 in all groups had no significant differences between each other (P 〉 0. 05 ). There were epithelial defect, congestion, inflammatory cell infiltration, edema and hyperplasia, vascular dilatation and scattered thrombi in the endometrium of non-invasive group and laparotomy group, but were more extensive in the endometrium of IUD placement side in the laparotomy group. Conclusions Compared with the conventional laparotomy IUD placement, the rat models with non-invasive IUD placement show significantly reduced peritoneal irritation, and may better reflect the clinic
