定形次全厚皮片移植术的临床应用

摘　　要：目的观察定形次全厚皮片移植技术修复创面的临床效果。方法 2004年8月～2012年6月,采用定形次全厚皮片移植术,修复45例整形或烧伤病人需植皮修复者,瘢痕切除后植皮者24例,其中手部9例,面部6例,足部4例,腘窝3例,颈部2例;烧伤后植皮者21例,其中手部13例,面部4例,足部1例,颈部3例。移植皮片面积最大22cm×15cm,最小4cm×6cm。结果 45例定形皮片中除3例有小面积皮片下血肿(经换药愈合2例,再次小范围植皮修复1例),其余均成活理想。术后6～12个月随访,各修复部位皮片缝合处瘢痕不明显,表面平坦,无明显的外表畸形,弹性良好,质地柔软。42例定形皮片色泽接近正常皮肤,3例定形皮片有不同程度色素沉着。功能恢复满意。结论定形次全厚皮片移植术是一种外观和功能均获得满意效果的创面修复方法,供区部位及大小不受限制。Objective To observe the wound repair effect of shaped sub-full thickness skin grafting tech-nology in wound repair .Methods From Aug.2004 to Jun.2012,45 patients received skin grafting using shaped sub-full thickness skin.There were 32 males and 13 females,aged from 1 to 54 years （mean,32.5 years）.Among the patients,24 patients received skin grafting following scar excision ,including 9 cases in hand,6 cases in face,4 cases in foot ,and 3 cases in popliteal region ,and 2 cases in cervical region;21 patients received skin grafting follow-ing burn injury,including 13 cases in hand,4 cases in face,1 case in foot,and 3 cases in cervical region .The skin grafting size ranged from 4cm ×6cm to 22cm ×15cm.Results Small hematoma was observed under the grafted skin in 3 patients,among whom,2 cases healed naturally after dressing change ,1 case underwent a second grafting in small range.The other 42 skin grafts were completely survived .After half a year to 1 year of follow-up,the appear-ance and function of these repaired skin gained ideal effects except varying degree of amorphous pigmentation in 3 cases.Conclusion The shaped sub-full thickness skin graft is an ideal technique to gain aesthetic repair of wounds.
