摘  要:目的:观察紫草对大鼠随意型皮瓣形态学及血清SOD、MDA、NO、TNF-α和IL-6的变化,探讨紫草的作用机制。方法:取大鼠随机分为3组,紫草组(外用100%紫草软膏0.1g/kg)、对照组(外用紫花烧伤膏,简称紫花组0.1g/kg)和模型组(外用凡士林0.1g/kg)。在大鼠背部头侧的大小约9cm×3cm的随意型皮瓣,2次/日涂药,观察大鼠皮瓣成活情况;比较术后1、2、3和7d血清SOD、MDA、NO、TNF-α和IL-6的变化并行组织形态学检查。结果:紫草的外用起效剂量为0.033g/kg,其剂量范围在0.033~3g/kg,皮瓣成活面积的百分比为:紫草组(51.92±15.48)%,紫花组(66.25±13.54),模型组(29.75±14.82)%,紫草量组皮瓣存活率明显高于模型组,与紫花组比较无统计学意义;紫草组血清SOD活性和NO含量较高,MDA、TNF-α和IL-6水平较低。组织学7d结果显示,模型组下段皮瓣组织真皮和毛囊有明显的变性和坏死。紫草组与紫花组下段皮瓣组织水肿、坏死及炎症细胞浸润比较轻,肉芽组织、成纤维细胞增生明显,有大量新生的毛细血管形成。结论:紫草能明显改善大鼠皮瓣血液循环,促进皮瓣成活,其机制可能与抗自由基损害、改善局部微循环、提高SOD与NO的含量、降低MDA、TNF-α和IL-6水平、减少炎症因子释放,改善氧化应激状态,减轻炎症反应有关。Objective: To observe the effect of Radix lithospermi on the survival of skin flaps in rats by histomorphology and SOD,MDA,NO,TNFα,IL-6 in serum,and further discusse its mechanisms.Methods: Wistar rats,were randomly divided into alfalfa group(external application of 100% Radix lithospermi0.1g / kg) 、control group(external application of alfalfa burn cream0.1g / kg) 、model group(external application of vaseline0.1g / kg).9cm × 3cm random skin flap with pedicle near the head was designed on the back of Wistar rats,2 times / d with surface drugs,and observed the survival condition of skin flaps.The change of serum SOD、MDA 、NO、TNF-α and IL-6 were compared at 1、2、3、7d after operation,and examined histomorphology.Results: The external use effective doses of Radix lithospermi was 0.033g / kg and its dosage range was from 0.0333g / kg to 3g / kg.The survival rate of Radix lithospermi group(51.92 ± 15.48) % was significantly higher than that of model group(29.75 ± 14.82) %,compared with control group(66.25 ± 13.54) %,there was no significant difference(P > 0.05).The level of serum SOD、NO in Radix lithospermi group and control group was higher than that in model group and the level of serum.MDA、TNF-α and IL-6 was lower than that in model group(P < 0.05).On 7 day after operation,skin flaps tissue edema、necrosis and inflammatory cell infiltration in Radix lithospermi group and control group was alleviated than that in model group.There was significantly proliferation of granuloma and fibroblast and formation of neonatal capillary.Conclusion: Radix lithospermi could significantly improve the blood supply of skin flaps,increase the survival rate of skin flaps in rats.Its mechanism may be associated with the anti-free-radical-damage action,improving local microcirculation,improving the content of SOD and NO,reducing the MDA,TNF-α and IL-6 inflammation factors,improving the oxidative stress status,reducing inflammation reactions.
