中医方药口服治疗溃疡性结肠炎随机对照试验的系统评价

摘　　要：目的:系统评价中医方药口服治疗溃疡性结肠炎(ulcerative colitis,UC)的疗效和安全性。方法:根据Cochrane系统评价方法,检索PubMed、CNKI、CBM、VIP、万方数据库(各数据库均从建库至2012年9月)中收录的中医方药口服治疗UC的临床随机对照试验研究;对符合条件的研究,资料提取和质量评价后,采用RevMan5.1软件进行Meta分析,并进行异质性检验、漏斗图分析。结果:①按照检索策略并取重后,共查到相关文献1393篇。通过阅读标题、摘要筛选后,共纳入23项研究,1923例患者。②Meta分析结果显示:单纯中医方药口服疗法治疗UC有着较好的疗效,显著优于西药阳性对照组治疗UC总有效率OR=5.14,95%CI[3.80,6.97]、中医证候疗效OR=2.29,95%CI[0.99,5.32]、肠黏膜的疗效OR=5.01,95%CI[0.56,44.71]、中医证候积分OR=-2.44,95%CI[-3.00,-1.87]、内镜指数变化OR=-0.48,95%CI[-1.01,0.04],并且不良反应发生率显著低于西药阳性对照组OR=0.06,95%CI[0.01,0.52]。结论:中医方药口服治疗UC有较好疗效和安全性,不良反应少。Objective： To evaluate the effectiveness and safety of method of chinese herbs oral therapy for treating UC.Methods： According to the Cochrane systematic review methods,retrieved clinical randomized,controlled trials of treating UC with method of chinese herbs oral therapy from PubMed,CNKI,CBM,VIP,Wanfang.The quality of studies meeting inclusion criteria was evaluated,the related data were extracted;Meta-analyses were performed by RevMan5.1 software,conducted tests for heterogeneity,Meta-analysis,sensitivity analysis,funnel plots analysis.Results： ① Collected 1393 related clinical controlled trials,fand the total sample size was 1923 cases.②Meta-analyses showed that： Compared with controlled group,total effective rate OR = 5.14,95% CI [3.80,6.97],TCM syndromes OR = 2.29,95% CI[0.99,5.32],Efficacy of the intestinal mucosa OR = 5.01,95% CI[0.56,44.71],TCM syndrome score OR =-2.44,95% CI[-3.00,-1.87],Endoscopic index changes OR =-0.48,95% CI[-1.01,0.04],Incidence of adverse reactions OR = 0.06,95% CI[0.01,0.52].Conclusion： Method of chinese herbs oral therapy for treating UC has a better efficacy and safety.
