摘 要:目的:探讨消化性溃疡患者的心理状况及健康教育干预前后对自然杀伤(natural killer cell,NK)细胞的影响,为消化性溃疡患者的健康教育提供参考依据.方法:对98例消化性溃疡患者采用症状自评量表(SCL-90)进行心理状况调查,给予相应的健康教育指导.在健康教育前后采用抑郁自评量表(self-rating depressive scale,SDS)及焦虑自评量表(self-rating anxiety scale,SAS)进行评定,比较分析患者对外周血NK细胞活性的影响.结果:98例消化性溃疡患者症状自评量表(S C L-90)中,总分(138.10±31.40)显著高于全国成人常模(t=2.348,P<0.05),并且焦虑(t=3.872,P<0.001)、敌对(t=2.269,P<0.05)和恐怖症状(t=3.167,P<0.001)均高于全国成人常模(P<0.05).经健康教育指导后,患者有关疾病及健康知识等方面的掌握程度较教育前有明显提高.通过SDS、SAS评定,患者健康教育4 wk末评分均有显著性下降(P<0.01).N K细胞活性19.31%±5.70%低于健康教育后25.25%±7.01%,差异具有统计学意义(t=3.35,P<0.01).结论:消化性溃疡患者存在明显的情绪障碍,常伴有细胞免疫功能异常,健康教育指导对预防控制疾病的进展,延缓并发症的发生具有重要意义.AIM: To investigate the psychological status of patients with peptic ulcer and to assess the impact of health education intervention on NK cells in these patients. METHODS: Ninety-eight patients with peptic ulcer were included in this study. Their mental status was assessed using the Symptom Check- list (SCL-90) for Mental Status, and appropriate health education and guidance were then given. The Self-Rating Depression Scale (SDS) and Self- Rating Anxiety Scale (SAS) were used to evalu- ate depression and anxiety between before and after health education and guidance. Peripheralblood natural killer (NK) cell activity was also compared. RESULTS: The total SCL-90 score (138.07 ± 31.33) for the 98 patients was significantly higher than the national adult norm (t = 2.345, P 〈0.05). Anxiety (t = 3.878, P 〈 0.001), hostility (t = 2.271, P 〈 0.05) and terror symptom scores (t = 3.166, P 〈 0.001) were also higher than the national adult norm. After health education and guidance, the patient-related disease and health knowledge and other aspects of mastery significantly im- proved compared to before education. SDS and SAS scores were significantly decreased 4 weeks after health education (both P 〈 0.01). NK cell activity was significantly higher after educa- tion than before education (19.23% ± 45.60% vs 24.65% ± 6.89%, t = 3.32, P 〈 0.01). CONCLUSION: Patients with peptic ulcer suffer from obvious emotional disorders and some- times immune dysfunction. Health education and guidance are important for preventing and controlling disease progression and delaying the occurrence of complications.
