应用鼻腔冲洗器冲洗鼻腔对慢性鼻-鼻窦炎患者鼻腔黏液纤毛功能的影响

摘　　要：目的观察应用鼻腔冲洗器冲洗鼻腔对慢性鼻-鼻窦炎患者鼻腔黏膜纤毛功能的影响。方法 60例单纯性慢性鼻-鼻窦炎患者,随机分为治疗组和对照组。治疗组用3%高渗盐水经鼻腔冲洗器冲洗鼻腔,对照组用3%高渗盐水经灌肠袋冲洗鼻腔,均为每日两次,连续冲洗14天,并辅以短期药物辅助治疗。分别于治疗前、治疗2周及疗后3月以糖精试验法检测鼻腔黏液纤毛清除率,比较两组鼻腔黏膜纤毛功能恢复情况和症状与体征的变化。结果治疗前2组的鼻腔黏液纤毛清除率差异无统计学意义;治疗2周后治疗组清除率为6.15±2.16mm/min,对照组为4.99士1.35mm/min,P<0.05,前者的症状改善程度也较为明显;疗后3月治疗组清除率为6.81±1.45mm/min,对照组为6.69±2.41mm/min,P>0.05。结论用鼻腔冲洗器冲洗鼻腔可有效促进鼻腔黏膜纤毛功能的早期恢复,对慢性鼻-鼻窦炎有辅助治疗作用。Objective To investigate the availability of nasal irrigating with nasal cavity irrigator on nasal mucociliary clearing function among patients with chronic rhinosinusitis. Methods Included in this study were 60 patients with chronic rhinosinusitis without nasal polyps developed, randomly divided into treatment group （TG） treated by nasal cavity irrigating with 3% hypertonic saline via nasal cavity irrigator and control group （CG） treated by nasal irrigation with 3% hypertonic saline via enema bag, two times one day for 14 days. Also, all these patients were supplemented with a short-term of routine drug therapy. Then, changes of symptoms and signs were investigated, and determination of muociliary clearance velocity （MCV） was carried out by saccharin test method, with a comparison of therapeutic effect and MCV improvement between the two groups done by the end of irrigating therapy and at the 3rd month after the local irrigating therapy ended respectively. Results There were no statistically significant difference in the mean value of MCV between the two groups as detected before treatment. However, it was shown that MCV of TG was 6.15±2.16 mm/min, significantly higher than that of CG （4.99±1.35mm/min, P〈0.05） by the end of irrigating therapy and followed with significantly symptoms and signs improvement among the cases of TG. Until the end of 3rd month after the local irrigating therapy ended, the difference of MCV between the two groups was shown with no statistical significance （6.81±1.45mm/min for TG and 6.69±2.41mm/min for CG, P〉0.05）. Conclusion Nasal irrigating therapy carried out by nasal irrigator can significantly improve nasal mucocilliary clearing function recovering at an earlier stage of illness, and this should be helpful for cases with chronic rhino sinusitis when combined as a supplementary treatment for them.
