零缺陷管理在生殖专科护理管理中的实践

摘　　要：目的探讨零缺陷管理理论在生殖专科护理管理中的应用与效果。方法通过对1501例不孕患者完成IUI治疗的Ij52周期中应用6西格玛管理理念开展以“零缺陷”为护理目标临床护理，实施以建立临床路径，规范档案管理、证件的多点核查与管理、“两查四对两确认”的精液处置制度和“五查两对”手术室核查等“零缺陷”服务管理措施。结果无1例患者发生不良并发症，未出现因护理因素而暂停治疗的现象，随防率100％。结论认为从抛弃“缺点难免论”、培养安全预警意识、完善护理安全制度、将护理工作流程再造和护理质量持续改进与护士培训相结合，建立护理安全预防系统，并落实安全告知的签字制度、重视安全知识宣教和督察、坚持“三全”动态管理，严格执行三级质量控制及完善激励与约束机制等方面加强护理工作的过程控制，可以实现护理安全零缺陷管理目标。Objective To explore the application and effect of zero defect management theories in nursing management in reproductive department.Methods 1301 cases with infertility received 1332 cycles of IUI treatment.We applied Six Sigma management philosophy to carry out clinical care aimed at ＂zero defects＂.We established many management measures, such as establishing a clinical pathway, file management, multi-point inspection and management for certificates, ＂two check, four match, two confirmation ＂ for husbands＇ semen and ＂five check ,two match ＂ for operation room in order to reach the goal of ＂zero defect＂. Results No adverse complications and on treatment suspend occurred due to the nursing factors. The follow-up rate was 100%. Conclusion we can realize management goal of zero defect through improving the prevention system of nursing safety, training security consciousness of early warning, improving the incentive and restraint mechanisms and strengthening processes control and quality control in nursing.
