肠道病毒71型感染手足口病合并神经系统损伤患儿的干预措施

摘　　要：目的探讨分析肠道病毒71型(EV71)感染手足口病合并神经系统损伤患者的护理干预措施,在加快患儿痊愈的同时缓解家属的负面情绪。方法回顾性分析2009-2012年收治的362例手足口病患儿所接受的综合护理,并对患儿的恢复情况、入院时与出院时患儿家长的心理状态进行分析。结果经过高质量的治疗与护理干预,无1例患儿死亡,存活率100.0%,患儿的平均退热时间为(24.1±7.2)h,口腔溃疡愈合时间为(53.6±6.5)h,住院时间为(5.4±1.7)d,出院时,23.3%患儿家属焦虑、29.9%抑郁、29.1%无助、31.3%愤怒、46.2%自责,其心理状态及SCL-90各因子评分均明显优于入院时的焦虑70.0%、抑郁46.5%、无助41.4%、愤怒55.3%、自责72.2%(P<0.05)。结论高质量的护理干预措施不仅能够提高EV71感染手足口病合并神经系统损伤患者的存活率,促进病情的恢复,并能够改善患儿家属的心理状态。OBJECTIVE To study the interventions to the EV71 type HFMD patients complicated with nervous system damage so as to accelerate the recovery of the patients as well as alleviate the negative emotions of their family members. METHODS A retrospective analysis was performed for the comprehensive nursing given to 362 EV71 type HFMD patients complicated with nervous system damage who were enrolled the hospital from 2009 to 2012, then the recovery of the patients and the mental state of their family members at the admission or the discharge were dynamically analyzed. RESULTS After the high-quality treatment and care interventions, none of the patients died while the survival rate was 100.0 %. The average cooling time for the children was （24.1 ±7.2） h, oral ulcer healing time （53.6±6.5） h, length of hospitalization time （5.4±1.7） d. The state of mind of the parents of the patients at the discharge （23.3 % were anxious, 29.9 % depressed, 29.1% helpless, 31.3 % angry, and 46. 2% self-blamed） and the SCL-90 factor scores were significantly better than those at the admission （70.0% were anxious, 46. 5% depressed, 41.4% helpless, 55. 3% angry, 72. 2% self-blamed）（P〈0, 05）. CONCLUSION High-quality nursing interventions can not only improve the survival rate of EV71 type HFMD patients complicated with nervous system damage and promote the recovery of the disease but also can improve psychological state of the family members of the children.
