摘　　要：目的调查天津市部分三级医院手卫生的现状,评价医务人员洗手效果,以降低医院感染率。方法通过隐蔽式观察了解医务人员手卫生依从性,手部细菌检测评价洗手效果。结果观察期间,手卫生指征平均1.91次,医务人员手卫生执行平均1.48次,依从率为77.23%;医务人员在出现手卫生指征后,采用洗手、擦手、戴手套的手卫生执行方式;全院范围内改善洗手设施,床旁应用含乙醇消毒剂前后进行手部细菌检测,第1次采样42份,合格26份,合格率61.90%,第2次采样42份,合格39份,合格率92.86%,第2次检测合格率高于第1次,差异有统计学意义(P<0.05)。结论通过改善手卫生设施,应用含乙醇的手消毒剂,提高医务人员手卫生知识,能提高手卫生依从性,达到降低医院感染的目的。OBJECTIVE To investigate hand hygienic status in some tertiary hospitals in Tianjin and evaluate the hand washing effect of the medical staff so as to reduce the incidence of nosocomial infections. METHODS The hand hygiene compliance of medical staff was observed by the concealment approach, and the hand washing effect was evaluated by the bacterial test for the hands. RESULTS During the observation period, the hand hygiene indications have appeared for 1.91 times, executed 1.48 times in average, and the average compliance rate was 77.23%. In case of the hand hygiene indications, the hand hygiene approaches including washing hands, wiping hands, wearing gloves were performed . The hand washing facilities were improved in the hospital wide, and the bacteria in the hands were tested after being disinfected with ethanol-containing disinfectants, of 42 cases sampled for the first time, 26 cases were qualified with the qualified rate of 61.90%; of 42 cases sampled for the second time, 39 cases were qualified with the qualified rate of 92. 86% ; the qualified rate of the second time test was higher than that of the first time test, the difference was significant（P〈0.05）. CONCLUSION The hand hygiene compliance can be improved through improving hand hygiene facilities, applying the ethanol-containing hand disin- fectants, and improving the awareness of medical staff of the hand hygiene.
