2008-2011年鲍氏不动杆菌的耐药性变迁分析

摘　　要：目的了解2008-2011年医院鲍氏不动杆菌(ABA)的临床分离及对常用抗菌药物的耐药性变迁趋势,为临床合理选用抗菌药物提供理论依据。方法回顾性分析2008-2011年临床分离的ABA药敏结果,采用Microscan全自动微生物分析仪进行细菌鉴定及药敏试验,个别抗菌药物药敏试验采用K-B法,结果判定采用NCCLS/CLSI标准。结果 2008-2011年ABA分离率分别为2.48%、5.25%、7.83%、8.17%,呈逐年升高趋势(P<0.05),ABA对三代头孢菌素的耐药率在2008-2011年逐年升高(P<0.05),对青霉素类、四代头孢菌素、单环β-内酰胺类、碳青霉烯类、β-内酰胺/β-内酰胺酶抑制剂、喹诺酮类、氨基糖苷类抗菌药物的耐药率在2008-2011年逐年升高(P<0.05)。结论医院ABA分离率与耐药性呈上升趋势,ABA对多种抗菌药物呈现高度耐药状态,应予以高度重视。OBJECTIVE To learn the clinical distribution and trends of antimicrobial resistance of Acinetobacter bau- mannii（ABA） isolates from 2008 to 2011 so as to provide the basis for reasonable application of antibiotics. METHODS All the isolates of A. baumannii were obtained from patients hospitalized from 2008 to 2011. The identification and antimicrobial susceptibility testing were performed by Microscan automatic of microorganism identification and antimicrobial susceptibility testing system. A few susceptibility testings were performed by the Kirby-Bauer method. The results were determined according to NCCLS/CLSI standards. RESULTS The isolation rates of A. baumannii isolated during 2008 -- 2011 were 2. 48%, 5. 25%, 7. 83%, and 8. 17%, respectively, showing an upward trend （P 〈 0. 05）. The drug resistant rate of the A. baumannii to the 3rd generation eephalosporins was gradually increasing from 2008 to 2011（P〈0.05）. The drug resistances rates to penicillins, 4th generation cephalosporins, monocyclic β-lactams, carbapenems, β-lactams/β-1actamase inhibitors, quinolones, and aminoglycosides were increasing year by year from 2008 to 2010（P〈0.05）. CONCLUSION Both the isolation rate and the drug resistance rate of the A. baumannii strains keep an upward trend, and the strains are highly resistant to multiple antibiotics, which should be attached great importance.
