闭合手法整复结合支架外固定治疗胫骨远端骨骺骨折

摘　　要：目的探讨及评价闭合手法复位结合支架外固定术治疗胫骨远端骨骺骨折的疗效。方法广州中医药大学附属佛山市中医院骨科自行总结的正骨十四法对胫骨远端骨骺骨折进行手法整复并支架外固定24例,Salter-Harris分型,Ⅰ型6例、Ⅱ型5例、Ⅲ型11例、Ⅳ2例。结果 24例患者21例获得随访,3例无联系方式而失访,随访6~18个月,平均11.6个月,骨折全部愈合。疗效评价按Ovadia评价标准：优18例,良3例,可0例,差0例。结论闭合手法整复结合支架外固定治疗胫骨远端骨骺骨折既是减轻发生层受到挤压的有效措施,又有利于骨骺愈合及发育。[ Objective ] To investigate the clinical effect of distal tibia physeal injuries treated with closed reduc- tion and external fixation. [ Methods ] 24 cases of distal tibia physeal injuries were treated with external fixation and closed reduction. All the fractures according to Saher-Harris classification, type l 6 cases, type II 5 cases, type ]][ 11 cases, typelV 2 cases. [Results] 21 cases were followed up for an average of 11.5 months （range from 6 to 18 months）, 3 eases lost to follow-up. All the fractures healed. 18 cases were graded as excellent, 3 as good, 0 as fair and 0 as poor according to Ovadia effect assessment evaluation criteria. [ Conclusion ] Closed reduction combined with external fixation for the treatment of the distal tibia physeal injuries can reduce the extrusion to occur layer, in favor of epiphyseal union and development.
