摘 要:目的 通过不同能量体外冲击波对动物骨不连的治疗 ,确定骨不连治疗的最佳能量范围。 方法 制作兔骨不连模型并随机分为A、B、C、D 4组。分别用 0 .33,0 .42 ,0 .5 4,0 .6 0mJ/mm2 的能量行骨不连断端体外冲击波治疗 ,其中A组 11只 ,B、C、D组各 12只 ;定期拍片、组织学检查及电镜检查。 结果 X线片显示B、C、D组动物经体外冲击波治疗 ,其骨不连愈合效果较A组好 ,统计学上差异有显著性意义 ;组织学和电镜检查亦显示B、C、D组骨不连断端有良好的骨不连愈合表现 ,而A组仍为纤维组织和慢性炎症表现。 结论 应用 0 .42~ 0 .5 4mJ/mm2 能量作为骨不连的治疗能量较为理想 ,这一研究结果为体外冲击波治疗骨不连的临床应用提供了进一步的理论依据。Objective To investigate the best energy range of shock wave through treating nonunions.Methods The rabbits were used to make nonunion models and grouped randomly into 4 groups, group A (11 rabbits), B,C and D (12 rabbits respectively), which were treated with extracorporeal shock wave for 0.33, 0.42, 0.54, 0.60 mJ/mm 2 respectively. Standardized radiographs, histology and transmission electron microscopy (TEM) examination were performed periodically. Results Twelve weeks after treatment by extracorporeal shock wave, the radiogrphy showed that nonunions in group B,C and D had reached radiographically bone union. But in group A, for the most part, still had persistent nonunion. The results of histology and TEM examination showed large quantity of osteoblasts to proliferate and differentiate and large trabeculae to form in nonunion sites. In group A, there was much chronic inflammation and fabric tissues. Conclusions The energy density for 0.42-0.54 mJ/mm 2 is effective and safe for treatment of nonunion. The results provide further theoretical foundation for extracorporeal shock wave treating nonunions.
