摘  要:目的探讨早期糖尿病肾病(DN)肾小球内皮功能紊乱的原因和特点,并评价棓丙酯对早期DN大鼠内皮功能紊乱的防治作用。方法选择SD大鼠31只,随机选取7只作为正常对照组,余24只应用链脲佐菌素腹腔注射建立DN模型,成功后随机分为模型组和治疗组,治疗组每日腹腔注射棓丙酯溶液。8周后PAS染色评价各组肾组织病理变化;试剂盒检测肾小球组织一氧化氮、血管性假性血友病因子(vWF)和尿白蛋白水平;NOS分型试剂盒检测肾小球内eNOS活性;氧化试剂盒检测肾小球内氧化应激水平。结果①与正常对照组比较,早期DN大鼠肾小球组织vWF明显升高;尿白蛋白含量明显增加,且尿白蛋白含量增加的程度与vWF的水平正相关(r=0.57,P<0.05);肾小球组织匀浆中超氧化物歧化酶(SOD)水平明显下降,丙二醛(MDA)水平明显升高;分类检测发现肾小球组织eNOS下降。②与模型组大鼠比较,治疗组血vWF显著下降、尿白蛋白水平显著降低、氧化应激指标显著好转、肾小球组织eNOS活性显著升高。结论早期DN大鼠尿白蛋白水平与肾小球血管内皮功能状态密切相关;肾小球内氧化应激水平可能是影响DN肾小球内皮功能状态的重要因素,其机制与其对肾小球内皮细胞eNOS活性的干预作用相关;棓丙酯可能通过缓解DN的氧化应激而改善肾小球血管内皮功能。Objective It is to investigate the mechanism of glomerular endothelial dysfunction, and evaluate the effect of propyl gallate on preventing this disturb in rat models with diabetic nephropathy. Methods 31 SD rats were chosen in which 7 rats were selected as normal control gorup, Streptozotocin (STZ.60 mg/kg) was administrated in the other rats to establish animal diabetic nephropathy model. The diabetic rats were randomly divided into diabetic model group and propyl gallate treated group. PAS staining was used to observe the pathological changes. Commercial kits were used to detect the activity of glomerular eNOS, NO, oxidative stress, vWF and urinary aibumin level. Results ①Compared with normal control group, glomerular vWF and urinary albumin significantly increased in diabetic rats, and the level of urinary albumin was correlated with the level of glomerular vWF. Levels of glomerular oxidative stress increased. Activities of glomerular eNOS significantly decreased when examined by NOS activity classifying kit. ②Compared with that of model group, the levels of vWF and urinary albumin were obviously lower, eNOS activity was higher, glomerular oxidative stress was improved in diabetic rats. Conclusion Urinary albumin is correlated with endothelial dysfunction in both early stage of DN animal model. Glomerular oxidative stress is a key factor in influencing glomerular endothelial function maybe via amodulating activity of eNOS. Propyl gallate can improve endothelial dysfunction maybe through reducing oxidative stress.
