双侧输卵管同时妊娠腹腔镜保守治疗1例

摘　　要：目的探讨临床罕见病例双侧输卵管妊娠的诊断与治疗方法。方法对1例双侧输卵管妊娠病例临床发现、治疗过程及相关文献进行分析。结果该病例属初级双侧输卵管同时异位妊娠,腹腔镜下双侧探查及开窗取胚术完成双侧输卵管妊娠的及时诊断与保守治疗。结论双侧输卵管妊娠的发病因素与单侧异位妊娠相似,腹腔镜探查是双侧输卵管妊娠诊断的金标准,手术是异位妊娠治疗的唯一途径,开窗取胚术可实现双侧输卵管妊娠的保守治疗。Objective A 31-year married women with menopause for 50d complaining vaginal bleeding for a week and right lower quadrant abdominal pain for 10h was admitted in emergency.The patient was diagnosed as ectopic pregnancy clinically and ultrasonagraphically.Laparoscopic exploration was performed and the contents of bilateral Fallopian tubes were removed complete by salpingotomy.Pathological examination confirmed the diagnosis of bilateral tubal pregnancy.The relevant literatures were reviewed and the etiology,clinical course,diagnosis and treatment of bilateral tubal pregnancy were discussed in the article.Laparoscopic bilateral exploration is the gold method for diagnosis and removing embryos with laparoscopic salpingotomy can achieve the conservative cure for bilateral tubal pregnancy.
