摘 要:目的研究湖北省十堰地区妊娠相关血浆蛋白A(PAPP-A)及其基因IVS6+95(C/G)多态性与急性冠状动脉综合征(ACS)的相关性。方法随机选择ACS患者219例,其中男性125例,女性94例,平均年龄63.25岁。健康对照者212例,其中男性114例,女性98例,平均年龄59.66岁。取血液标本,应用荧光探针聚合酶链反应和基因芯片技术对其PAPP-A基因IVS6+95(C/G)多态性进行基因分型,用电化学发光法检测PAPP-A浓度。结果 ACS组PAPP-A显著高于对照组[(45.92±18.39)pg/mL vs(4.11±0.83)pg/mL],差异有显著统计学意义(P<0.01);亚组急性心肌梗死(AMI)组[(82.42±26.41)pg/mL]和不稳定心绞痛(UAP)组[(30.52±12.62)pg/mL]血浆PAPP-A都显著高于对照组,差异有显著统计学意义(P<0.01),AMI组与UAP组相比差异也有显著统计学意义(P<0.01)。ACS组GG、GC、CC基因型频率分别为43.38%、41.09%、15.53%,对照组GG、GC、CC基因型频率分别为50.00%、43.40%、6.60%,差异具有统计学意义(P<0.05)。G、C等位基因频率在ACS组和对照组分别为63.93%、36.07%和71.70%、28.30%,差异也具有统计学意义(P<0.05)。在ACS组和对照组,各类基因型人群PAPP-A浓度差异无统计学意义(P>0.05)。结论 PAPP-A及其基因IVS6+95(C/G)多态性与ACS相关,C等位基因和CC基因型是ACS发病的易感危险因素之一。Objective To study the relationship between pregnancy-associated plasma protein-A(PAPP-A) with its gene IVS6 + 95(C/G) polymorphism and acute coronary syndrome(ACS). Methods A total of 219 ACS patient were enrolled, included 125 males and 94 females, with a mean age of 63.25. A total of 212 healthy as controls were selected, included 124 males and 98 females with a mean age of 59.66. The PAPP-A gene IVS6 + 95(C/G) polymorphism genotyping was detected by fluorescent probes polymerase chain reaction(PCR) and mieroarray technique, the PAPP-A concentrations were detected by electrochemiluminescenee method. Results The PAPP-A of ACS group was significantly higher than that of the control group[(45.92 + 18.39) pg/mL vs (4.11 ± 0.83) pg/mL](P〈 0.01). The PAPP-A in acute myocardial infarction (AMI) group [(82.42 ± 26.41) pg/mL] and unstable angina pectoris (UAP) group [(30.52 ± 12.62) pg/mL] were significantly higher than that of the control group(P〈 0.01), the difference in PAPP-A between AMI group and UAP group was statistically significant(P 〈 0.01). The GG, GC and CC genotype frequencies in ACS group were 43.38 %, 41.09 %, 15.53 %, respectively, and GG, GC and CC genotype frequencies in the control group were 50.00 %, 43.40 %, 6.60 %, respectively, with statistically significant difference between 2 groups(P 〈 0.05). The G, C allele frequencies in ACS group and the control group were 63.93 %, 36.07 % and 71.70 %, 28.30 %, respectively, and the difference between 2 groups was statistically significant(P 〈 0.05). There was no significant difference in PAPP-A concentration among the genotypes in all groups(P 〉 0.05). Conclusion It is demonstrated that PAPP-A and its gene IVS6 + 95 (C/G) polymorphism are associated with ACS, the C allele and CC genotype are risk- predisposing factors for ACS.
