运动训练联合心理激励对慢性阻塞性肺疾病患者生活质量的影响

摘　　要：目的探讨肺康复运动训练联合心理激励对慢性阻塞性肺疾病(COPD)患者生活质量的影响。方法选取中重度COPD稳定期住院患者120例,随机分为试验组60人和对照组60人,对照组接受常规治疗1年,试验组在对照组基础上进行呼吸肌运动锻炼和肢体运动锻炼的同时融入心理激励元素,对比两组间康复训练前后生活质量、6 min步行距离、肺功能及焦虑、抑郁情况。结果实验组患者康复锻炼后生活质量明显提高(P<0.01),6 min步行距离明显延长,焦虑、抑郁明显好转,和对照组相比差异具有统计学意义(P<0.05)。实验组康复后肺功能较对照组无明显减损。结论运动锻炼联合心理激励的综合肺康复训练方案可改善患者躯体状况和心理状况,提升其社会适应能力,提高生活质量。Objective To explore the effect of rehabilitation exercise combined with encouragement on the quality of life of patients with chronic obstructive pulmonary disease （COPD）. Methods A total of 120 inpatients with moderate or severe COPD in stable conditions were randomized equally into experimental and control groups. The patients in both groups received routine treatment for 1 year, and those in the experimental group had additional respiratory muscle and limb exercises with encouragement. The quality of life, 6-min walking distance, lung function, anxiety and depression levels were compared between the two groups before and after the treatments. Results The quality of life was improved significantly in the experimental group after the treatments （P〈0.01） with obviously increased 6-min walking distance and lowered levels of anxiety and depression in comparison with those in the control group （P〈0.05）. Compared with the control group, the patients in the experimental group showed also significantly improved lung functions. Conclusion Comprehensive pulmonary rehabilitation training program with exercise combined with encouragement can improve the physical and emotional state, social adapt ability and the quality of life of patients with COPD.
