摘  要:目的探讨高血压病患者冠状动脉内皮功能超声表现与血清人类软骨糖蛋白-39(HCgp-39)水平的关系及意义。方法应用经胸超声结合冷加压试验检测75例高血压病患者(高血压病组)和26例正常对照组的冠状动脉左主干内径变化率(△DLM)及前降支中远段峰值血流速度变化率(△VLAD)以无创性评价冠状动脉内皮功能,测定血清HCgp-39水平。分析△DLM和△VLAD与血清HCgp-39水平之间的关系。结果高血压病组与正常对照组基线左主干内径及前降支中远段峰值血流速度比较无统计学意义,但高血压病组有着更高血清HCgp-39水平(P<0.01)。冷加压试验后高血压病组△DLM和△VLAD均明显低于正常对照组(均P<0.01)。高血压病组△DLM和△VLAD分别与血清HCgp-39水平呈直线负相关(r值分别为-0.412和-0.631,均P<0.01);多因素分析控制传统心血管危险因素后,上述相关关系仍然存在(β′分别为-0.349和-0.401,均P<0.01)。结论高血压病患者冠状动脉内皮功能障碍与血清HCgp-39水平呈显著负相关,此相关性独立于其他相关危险因素。Objective To investigate the correlation between human cartilage glycoprotein 39 (HCgp-39) level and endothelial function of coronary using ultrasound detection in patients with essential hypertension and its clinical significance. Methods The diameter of the left main coronary artery ( LM ) and the peak flow velocity of left anterior descending coronary artery(LAD) in 75 essential hypertension patients and 26 healthy volunteers were measured by ultrasound before and after cold presseor test, and the percent changes of LM diameter(ADr~) and LAD peak flow velocity(AVLAD) were calculated. The level of serum HCgp-39 was detected using enzyme-linked immunosorbent assay. Bivariate correlation analysis and multiple linear regression were applied to show the correlation of endothelial function of coronary and serum HCgp-39 levels. Results No difference of the diameters of LM and peak flow velocity of LAD was found at baseline in the two groups. The serum level of HCgp-39 was significantly increased in hypertension group in comparison with those in the control group(P〈0.01 ), but the aDm and A VLA~ were significantly lower(P〈0.01 ). The A DI~ and A VLAD were negatively correlated with serum HCgp-39 level, respectively (r=-0.412, -0.631, P〈0.01 ). And the correlations was still kept negative after the adjustment with other factors under the control of stepwise multiple linear regression mode( β=-0.349,-0.401, P〈0.01 ). Conclusion The endothelial dysfunction of coronary positively correlated with serum HCgp-39 level in patients with essential hypertension, before and after adjustment with other factors.
