中晚孕期系统超声筛查胎儿畸形的价值

摘　　要：目的探讨开展中、晚孕期两次系统超声在筛查胎儿先天性畸形中的应用价值。方法2008年1月起，对来我院行产前系统超声检查的13874名孕妇，均于中（孕20～24周）、晚孕（30～34周）期各进行一次系统超声检查。每位孕妇均详细登记，并对其异常胎儿进行追踪随访，所有胎儿资料均存放于工作站内以便行系统评估和统计。结果在13874例孕妇中，经产后随访证实畸形胎儿384例，畸形发生率2．77％（384／13874）。孕20．24周时检出畸形胎儿315例，检出率82．03％（315／384），孕30～34周时新检出畸形胎儿43例，检出率11．20％（43／384）。两次系统超声检查共检出畸形胎儿358例，畸形胎儿检出率提高至93．23％（358／384），漏诊26例，漏诊率6．77％（26／384）。结论于中、晚孕期各行一次系统超声检查可显著提高畸形胎儿的检出率，具有重要的临床价值。Objective To evaluate the clinical value of double systemic ultrasound screening at the second and the third trimesters for fetal congenital malformations. Methods From January 2008 to December 2011, data of double systemic ultrasound examinations performed to 13 874 pregnant women during the time of the second trimester （20~24 weeks）, and the third trimester （30-34 weeks） were collected and analyzed. Information of each pregnant woman was registered and their fetals were followed up. All fetal data were stored in workstation entirely for evaluation and statistics analysis. Results There were 384 cases of defect fetal, the deformity rate was 2.77%（384/13 874）. In these cases, 315 were detected during the 20~24 th week, the detection rate was 82.03% （315/384）, 43 cases were detected during the 30N34 th week, the detection rate was 11.20% （43/384）. Altogether 358 cases were detected correctly with double examination. The overall detection rate of double detection was 93.23%（358/384）, which was significant higher than one time examination （P〈0.01）. Twenty-six cases were undetected, the undetected rate was 6.77% （26/384）. Conclusion Double systemic ultrasound during the second-trimester and the third-trimester is valuable in clinic diagnosis because of higher overall detection rate in defect fetuses diagnosis.
