摘  要:目的检测不同病程糖尿病大鼠肾脏背向散射积分(IBS),同时观察纤维化指标,探讨IBS与肾脏纤维化的关系。方法腹腔注射链尿佐菌素(65mg/kg)建立SD大鼠糖尿病模型,于第4、12、24周测定肾皮质和肾髓质的IBS%,Masson染色观察血管周围胶原面积(PVCA)、肾小球胶原沉积评分(GCDS)、肾小管间质病变评分(TIDS),肾脏胶原蛋白(CollagenⅠ、CollagenⅢ)免疫组织化学染色。心肌光镜与透射电镜观察,并与同龄正常大鼠进行比较。结果①与对照组相比:糖尿病组肾皮质、肾髓质的IBS%明显增高(P<0.05);糖尿病组肾脏纤维化指标PVCA、GCDS、TIDS、CollagenⅠ、CollagenⅢ的表达均明显增高(P<0.05);②肾皮质与肾髓质IBS%与PVCA、GCDS、TIDS、CollagenⅠ、CollagenⅢ表达呈正相关(P<0.05);③电镜发现糖尿病组肾小球毛细血管基底膜弥漫性增厚,间质胶原增生。结论糖尿病大鼠存在明显的肾脏纤维化,通过监测IBS可以监测糖尿病大鼠肾脏组织纤维化的程度。Objective To investigate the relation between renal ultrasonic integrated backscatter (IBS) and renal fibrosis of diabetic rats in the different stages. Methods The diabetes mellitus in healthy male SpragueDawley rats was induced by a single injection of streptozotocin (STZ, Sigma) into intraperitoneal at a dose of 65 mg/kg body weight. The renal cor tex and medulla IBS by IBS were determined in rats of 4, 12 and 24 weeks after DM was induced. Masson's trichrome was used in the study of perivascular collagen area (PVCA), glomerular collagen deposition score (GCDS), tubular inter stitial disease score (TIDS) in kidney interstitial tissue. The type Ⅰand Ⅲ collagen were evaluated by qualitative and semiquantitative immunohistochemical staining, while, the agematched normal rats were served as control group. Renal pathomorphology changes were observed with light microscope and transmission electron microscope. Results (1) The re nal cortex and medulla IBS% of were significantly higher in diabetic rats than those in the control group ( P 〈0.05). Compared with the control group, perivascular collagen area (PVCA), glomerular collagen deposition score (GCDS), tu bular interstitial disease score (TIDS, and the expression of the type Ⅰ and Ⅲ collagen in renal were increased in diabetic rats ( P 〈0.05) (2) The correlation between IBS% and PVCA, GCDS, TIDS, Collagen I and Collagen Ⅲ were signifi cant ( P 〈0.05) (3) Diffuse thickening of glomerular capillary basement membrane and interstitial collagen hyperplasia in diabetic rats were observed by electron microscope. Conclusion There is an obvious nephritic fibrosis in diabetic rats. Ul trasonic myocardial IBS has been demonstrated to reflect the degree of renal fibrosis in diabetic rats.
