舒芬太尼联合地佐辛对老年全麻手术患者胰岛素抵抗和hs-CRP的影响

摘　　要：目的探讨舒芬太尼联合地佐辛对老年全麻手术患者胰岛素抵抗和超敏C反应蛋白(hs-CRP)的影响。方法 80例老年全麻手术患者随机分为两组,观察组40例,对照组40例,对照组给予盐酸昂丹司琼0.2mg/kg+舒芬太尼2.5g/kg镇痛泵,观察组给予盐酸昂丹司琼0.2mg/kg+舒芬太尼1.5g/kg+地佐辛0.2mg/kg镇痛泵。测定胰岛素抵抗指数和血清hs-CRP。结果两组患者术后2h,12h,24h的VAS和Ramsay相当(P>0.05)。两组在术后胰岛素抵抗与hs-CRP均有明显升高,但对照组比观察组升高更明显(P<0.05)。结论舒芬太尼联合地佐辛可以明显减轻老年全麻手术患者胰岛素抵抗和hs-CRP。Objective To study sufentanil plus dezocine on insulin resistance and high-sensitivity C-reactive protein（hs-CRP） for anesthesia in elderly surgical patients.Methods 80 elderly patients with surgery under general anesthesia were randomly divided into two groups,the observation group（n=40 cases） and the control group（n=40 cases）.Control group received ondansetron hydrochloride Joan 0.2 mg/kg ＋ sufentanil 2.5 g/kg through analgesia pump,observation group were given ondansetron hydrochloride 0.2 mg/kg ＋ sufentanil 1.5 g/kg ＋ dezocine the 0.2mg/kg through analgesia pump.Serum insulin resistance index and hs-CRP were determinated.Results Patients in both groups VAS and Ramsay after 2h,12h,24h were equivalent（P＆gt; 0.05）.The two groups in postoperative insulin resistance and hs-CRP were significantly higher,but the control group increased more significantly than in the observation group（P＆lt; 0.05）.Conclusion Sufentanil plus dezocine can significantly decrease serum insulin resistance and hs-CRP for anesthesia in elderly surgical patients.
