摘  要:目的 探讨微波烘疗对肢体慢性淋巴水肿纤维化的影响。 方法 应用 ABC免疫组化及原位杂交技术观察患肢皮肤组织 TGF-β和 、 型前胶原 m RNA及多肽含量变化。 结果 烘疗前 TGF-β1 多肽位于表皮棘层、颗粒层细胞 ,真皮及皮下组织中见大量 、 型胶原纤维并有 TGF-β和 、 型前胶原 m RNA表达增加 ;烘疗后表皮细胞 TGF-β1 阳性染色变浅 , 、 型前胶原纤维变细 ,TGF-β和 、 型前胶原 m RNA表达降低。 结论 肢体慢性淋巴水肿纤维化发生 ,与相关基因TGF-β的过量表达导致 、 型胶原等细胞外基质合成增加和沉积有关 ,微波烘疗可以抑制 TGF-β基因表达及多肽的合成 ,通过减少 、 型前胶原 mObjective To observe the effects of microwave heating on skin fibrosis in chronic extremity lymphedema. Methods Skin specimens from 8 cases of chronic limb lymphedema were tested by in situ hybridization (ISH) combined with avidin biotin peroxidase (ABC) immunohistochemistry for detection of TGF β, procollagen Ⅰ, procollagen Ⅲ mRNAs and corresponding peptides expressions. Results It was discovered that expressions of TGF β 1 peptide were located at the spinous and granular layer of the epidermal cells with a great amount of dermal collagen Ⅰ,Ⅲ formation in accordance with high expressions of TGF β, procollagen Ⅰ, procollagen Ⅲ mRNAs in the dermal and subcutaneous tissue fibroblasts. After microwave heating treatment, the epidermal expression of TGF β 1 and relative TGF β,procollagen Ⅰ, procollagen Ⅲ mRNAs expressions in dermal fibroblasts were greatly reduced. The smaller calibre of collagen fibers after microwave heating was also observed. Conclusions It is indicated that fibrosis in lymphedema is resulted from over expressions of relevant genes like TGF β and subsequent extracellular matrixes (ECM) syntheses and deposition. Microwave heating can reduce fibroblast expressions of TGF β, procollagen Ⅰ, procollagen Ⅲ mRNAs as well as TGF β peptide synthesis, inhibiting ECM syntheses and deposition and finally reverse the skin fibrosis process.
