摘 要:目的 探讨参附注射液对脊髓缺血性损伤保护作用的剂量效应关系。方法 2 4只雄性新西兰大白兔 ,随机分为四组 :A组 (n =6 ) ,单纯缺血再灌注组 ,B组 (n =6 )、C组 (n =6 )和D组 (n=6 )分别于缺血前 30min内持续恒速静脉输入参附注射液 5ml·kg-1、10ml·kg-1、2 0ml·kg-1;采用肾下主动脉阻断法造成脊髓缺血 (2 0min) ;术后观察神经功能变化并记录再灌注 1h、4h、8h、12h、2 4h和 48h神经功能评分 ,再灌注 48h处死动物后取脊髓 (L5~ 7)标本行病理学观察。结果 术后神经功能评分除再灌注 1hD组与A组之间无显著性差异 ,其余各时间点B组、C组和D组均明显高于A组 (P <0 0 5 ) ,B组、C组和D组间则无显著性差异 (P >0 0 5 ) ,且 48h时各组神经功能评分分别为A组 0 5±0 8、B组 3 2± 0 9(P <0 0 0 1)、C组 3 0± 1 0 ,(P <0 0 0 1)和D组 2 7± 0 8(P <0 0 5 ) ;再灌注 48h脊髓前角正常运动神经元计数B组 (78± 2 5 ,P <0 0 0 1)、C组 (41± 2 3,P <0 0 5 )和D组 (42± 2 7,P <0 0 5 )均高于A(8± 7)组 ,B组与C组、D组间亦有显著性差异 (P <0 0 5 )。结论 参附注射液对脊髓缺血性损伤有保护作用 ,但无明显剂量效应关系。Objective To investigate the dose response effects of Shenfu injection, an intravenous drug made from traditional Chinese herbs, on the ischemic spinal cord injury in rabbits Methods New Zealand white rabbits were anesthetized with halothane and spinal cord ischemia was induced with 20 min infrarenal aortic occlusion Animals were randomly allocated to 4 groups, in group A(n=6) without pharmacologic intervention, in group B (n=6) ,group C (n=6) and group D (n=6) Shenfu injection 5,10 and 20ml·kg -1 were infused intravenously at a constant rate within 30min before the aortic cross clamping, respectively Neurologic status was scored by the Tarlov system 1,4,8,12,24 and 48h following reperfusion The animals were sacrificed 48h following reperfusion to sample the spinal cord (L5 7) immediately for histopathologic study Results All animals survived during the experiment Compared with that in group A, the neurologic deficit score (NDS) increased significantly at each observing time in group B, group C and group D (P<0 05), except for that 1h following reperfusion in group D (P>0 05), but there was no significant difference between group B, C and D NDS in group A,B,C and D 48h after reperfusion were 0 5±0 8,3 2±0 9(P<0 001),3 0±1 0( P<0 001)and 2 7±0 8( P<0 05)respectively The amount of normal neurons at anterior spinal cord of group B(78±25, P<0 001),C(41±23,P<0 05 ) and D(42±27, P<0 05) was more than that of group A(8±7) 48h following reperfusion (P<0 05) Conclusion Shenfu injection can effectively reduce neurologic injury induced by spinal cord ischemia and reperfusion without dose response effect
