纽曼系统护理模式对冠脉造影患者术前焦虑及术后并发症的影响

摘　　要：目的 探讨纽曼系统护理模式对冠脉造影患者术前焦虑及术后并发症的影响.方法 将150例冠脉造影患者采用随机数字表法分为干预组及对照组各75例,对照组采用常规护理措施,干预组应用纽曼系统模式进行护理,干预前后采用汉密尔顿焦虑量表（HAMA）进行评分,对患者并发症以及患者和家属满意度进行调查.结果 干预组HAMA得分为（13.10 ±6.08）分,较治疗前（23.44±3.90）分明显降低,且明显低于对照组（20.87±10.36）分,两组比较差异有统计学意义（t=0.621,P＜0.05）；干预组心律失常、心绞痛、低血压、皮下血肿、发热、24h内排尿困难以及对造影剂过敏的发生例数分别为2,0,1,0,1,0,1例,对照组为8,6,5,5,7,6,7例,两组比较差异有统计学意义（x2值分别为3.857,6.250,2.778,5.172,4.754,6.250;P ＜0.05）；干预组患者术后家属总体满意度明显优于对照组（Z=3.635,P＜0.05）.结论 纽曼系统护理模式可有效控制冠脉造影患者术前焦虑情况,降低术后并发症,提高患者及家属的满意度.Objective To investigate Neuman systems model of care for coronary angiography patients with preoperative anxiety and postoperative complications.Methods Totals of 150 cases of coronary angiography were randomly divided into intervention group and control group,with 75 cases each group.Control group received the usual care measures,while intervention group received Neuman systems model for care.Anxiety scores,incidence of postoperative complications and patient and family satisfaction were surveyed.Results After the intervention,the HAMA score of intervention group was （13.10 ± 6.08）,which was lower than the before which was（20.87 ± 10.36）,and it was also significantly lower than the same time of control group which was （20.87 ± 10.36）,and the difference was statistically significant （t =0.621,P ＜ 0.05）.The incidence of complications in intervention group such as the arrhythmia （2 vs 8）,angina （0 vs 6）,hypotension （1 vs 5）,subcutaneous hematoma （0 vs 5）,fever （1 vs 7）,24 h of difficulty urinating（0 vs 6）,and allergy to contrast agents （1 vs 7） were significantly less than that in control group （x2 =3.857,6.250,2.778,5.172,4.754,6.250 ;P ＜ 0.05）.The satisfaction of families in intervention group was better than that in control group （z =3.635,P ＜ 0.05）.Conclusions The Newran system care model can effectively control coronary angiography patients with preoperative anxiety,reduce postoperative complications and improve the satisfaction of patients and their families.
