摘 要:目的研究高强度神经节丛(GP)刺激对正常犬心室电生理性质的影响。方法取犬12只,分别经左、右侧第五肋间隙开胸,暴露心脏,将2根多极电生理导管分别沿心尖部至心底部缝于左、右心室游离壁各记录4个部位的局部电图,以自制Ag-AgCl电极记录左、右心室游离壁心尖部、心底部以及心尖部-心底部之间3个部位的单相动作电位,分别在基础状态和左、右以及双侧高强度GP刺激(降低窦性心率50%或出现2∶1房室传导阻滞)时测定心室各部位有效不应期(ERP)、动作电位时程(APD)、APD交替和构建APD回复曲线。结果高强度GP刺激显著延长心室各部位ERP及APD并降低ERP及APD的空间离散度(P均<0.05)。高强度GP刺激使心室APD回复曲线斜率及其空间离散度变小,APD交替延迟发生(P均<0.05)。结论高强度GP刺激可能具有抑制室性心律失常的作用。Objective To investigate the effect of high-intensity GP stimulation (GPS) on ventricular electrophysiologi- cal properties of normal heart in canine. Methods In 12 adult healthy mongrel dogs, both left and right thoracotomies were performed at the 5th intercostal space to expose the heart. Two multi-electrode catheters were sutured to record electro- grams at 8 epicardial sites from the apex to base in left and right ventricular free walls. A custom-made Ag-AgCl electrode was used to record monophasic action potentials from the epicardial surface of the left and right ventricular free walls at 6 sites. Ventricular effective refractory period (ERP), action potential duration (APD), electrical ahernans and dynamic APD restitution were measured at baseline and during high-intensity GPS ( the strength for high-intensity GPS was set at the voltage level causing a 50% decrease of sinus rate or 2:1 atrioventricular block). Results High-intensity GPS signifi- cantly prolonged ventricular ERP and APD at each site and reduced their spatial dispersion (all P〈O. 05). High-intensity GPS decreased the slope of APD restitution curves and its spatial dispersion as well as suppressed APD alternans ( all P〈0. 05). Conclusions High-intensity GPS may protect against ventr/cular arrhythmogenesis.
