摘  要:目的 探讨应用构建细菌16S rRNA基因文库的方法,明确健康育龄期妇女阴道菌群构成比例,并建立优势菌参考范围.方法 2010年10月~12月在首都医科大学附属北京友谊医院采集健康育龄期妇女阴道分泌物标本,共30例,一部分用于涂片革兰染色,另一部分用于提取细菌基因组DNA,构建细菌16S rRNA基因文库.同时,应用普通PCR技术扩增标本中的阴道加德纳菌、阴道阿托波菌、乳酸杆菌等7种阴道常见细菌,确定各菌种检出率.结果 健康育龄期妇女阴道分泌物革兰染色可见涂片中以革兰阳性大杆菌为主,仅有极少量革兰染色不定或阴性小杆菌.文库构建结果显示健康妇女阴道菌群构成单一,为1~3种细菌,卷曲乳酸杆菌占绝对优势,惰性乳酸杆菌所占比例较高,以及极少量其他细菌.卷曲乳酸杆菌95%参考值范围为50%~100%,惰性乳酸杆菌95%参考值范围为4%~44%.普通PCR结果显示7种细菌均可检出,但检出率明显不同,乳酸杆菌的检出率达到100%,而其他细菌检出率低于30%.结论 健康育龄期妇女阴道分泌物菌群表现为以乳酸杆菌占绝对优势、尤以卷曲乳酸杆菌为主、同时多种细菌共同存在的一种稳态,该参考范围的建立对于机体状态及相关疾病评估有重要意义.Objective The method of construction 16S rRNA library was used to illuminate the bacterial characteristics of healthy child-bearing period women, and reference range of predominant bacteria was further established. Methods 30 vagi- nal secretion samples of healthy child-bearing women were collected from Oct to Dec. 2010 in Beijing Friendship Hospital, Capital Medical University. One part were for Gram stain and another part were for the construction of 16S rRNA library. Besides,7 species were detected with specific PCR to know their prevalance. Results Healthy fertile women had 1 to 3 vagi- nal bacterial species (phylotypes) in each sample,as detected by broad-range PCR of 16S rDNA,and lactobacillus species,espeeially Lactobacillus crispatus,were the predominant bacteria. 95% reference range of Lactobacillus crispatus was 50 ~ 100 %, and that of Laetobacillus iners was 4 % ~ 44 %. Seven species could be detected, however, the positive rates differ from each other,with that of lactobacillus 100% and lower than 30% of other species. Conciosion Vaginal flora predominant with Lactobacillus crispatus was a normal status. The establishing of normal reference range of healthy fertile women is important for the evaluation of related diseases.
