白血病化疗患者骨髓造血功能恢复的评估意义

摘　　要：目的 研究急性非淋巴细胞白血病（ANLL）患者的网织血小板水平在化疗过程中的变化规律,探讨网织血小板水平在ANLL患者化疗期间骨髓造血功能恢复中的评估意义.方法 收集ANLL患者23例,年龄16～70岁.用Sysmex XE-2100全自动血细胞分析仪检测23例ANLL患者（ANLL组）化疗前、骨髓抑制期、骨髓恢复期及化疗后血小板计数（PLT）、网织血小板绝对值（RP）、网织血小板百分数（RP%）,与30例健康人（对照组）进行比较,并对结果进行统计学分析.结果 ANLL患者RP,RP%和PLT在疾病的不同状态下检测的结果是不同的,ANLL组化疗前PLT和RP低于对照组（t=4.837,13.930,P均〈0.05）;RP%与对照组比较,无明显差异（t=1.023,P〉0.05）;骨髓恢复期RP和RP%高于骨髓抑制期及化疗后（t=0.220～1.040,P均〈0.05）,而PLT计数无显著改变（t=0.029～0.987,P均〉0.05）;ANLL患者RP%与PLT呈负相关（r=-0.519,P〈0.05）.结论 外周血RP和RP%是反映ANLL化疗后骨髓造血功能恢复的一个早期而且敏感的指标,与PLT比较,RP能更有效地反映ANLL患者的骨髓造血功能是否恢复,以指导临床治疗,评估预后.Objective To evaluate the variation of reticulated platelet in acute non-lymphocytic leukemia （ANLL） patients during chemotherapy and discuss its significance in patients＇ recovery of bone marrow hematopoietic function. Methods The 23 ANLL patients aged 16~70 years old. Sysmex XE-2100 automated hematology analyzer detected 23 cases of patients with ANLL （ANLL group） before chemotherapy, bone marrow suppression period, recovering from bone marrow after chemotherapy platelet count （PLT）, the reticulated platelet absolute （RP）, reticulated platelet percentage （RP% ）, compared with 30 healthy people （control group）,and the results were statistically analyzed. Results ANLL patients with RP and RP%, the test results of the PLT in the different states of the disease was different, ANLL group chemotherapy before PLT RP lower than the control group（t=4. 837,13. 930 ,P〈0.05） ,and the RP% with control group,no significant difference （t =1.023, P〉0.05） ;recovering from bone marrow RP and RP% higher than bone marrow suppression hematopoietie function and chemotherapy （t=0. 220~1. 040, P〈0. 05）, and PLT counts did not changesignificantly （t= 0. 029 ~0. 987, P：〉 0.05）. The ANLL patients RP% PLT was negatively correlated （r= -0.519, P〈0. 05）. Conclusion The RP, RP% in the different stages of ANLL are different. Compared with PLT,RP is better to indicate the bone marrow recovery and so as to guide the clinical treatment.
