摘　　要：目的编制针对军队慢性病患者连续护理认知水平的测评系列工具,为连续护理的实施提供参考。方法以"心血管疾病患者连续护理认知水平测评量表"为"蓝本",采用文献分析法、小组讨论法、Delphi法,结合军队卫生保健实际和军队医改保障对象特点,编制包括心血管疾病、肿瘤、呼吸系统疾病、消化系统疾病、糖尿病、骨科疾病6大慢性病的军队慢性病患者连续护理认知测评系列问卷,分别对其信效度进行检验,并选定1 200例患者进行实证研究。结果共形成6个独立问卷,测评指标包括治疗知识、康复需求、社会生活需求3个维度,信效度检验均符合要求。结论军队患者6大慢性病连续护理认知测评工具库科学、合理、实用,可作为军队慢性病护理管理的有效测评工具推广使用。Objective To develop a series of assessment tools to assess the cognitive level of continuous nursing among patients with chronic diseases in army,so as to provide references for the implementation of continuous nursing.Methods Based on the Questionnaire of Cognitive Level on Continuous Nursing for Cardiovascular Patients,literature analysis,group discussion,Delphi method were introduced combining with the military health care actual situation and the characteristics of patients who recieved the service of military health care reform.The questionnaires of cognitive assessment on continuous nursing of six chronic diseases were made,and their reliabilities and validities were tested.The six chronic diseases included cardiovascular disease,cancer,respiratory disease,digestive disease,diabetes and orthopedic disease.A total of 1 200 patients were selected and received empirical research.Results Six dependent questionnaires were developed.Assessment indicators included three dimensions of therapy related knowledge,rehabilitation needs and social needs.Reliabilities and validities all met the requirement.Conclusion The assessment tools of cognition of continuous nursing for chronic patients are scientific,reasonable and practical,and can be used as effective assessment tools for the nursing management of military chronic diseases.
