摘　　要：目的探讨FOCUS-PDCA程序降低住院患者非计划性拔管发生率的应用效果。方法运用FOCUS-PDCA程序的发现、组织、澄清、理解、选择、计划、实施、检查和执行9个步骤分析非计划性拔管发生原因,制订改进措施。结果 2012年年度非计划性拔管发生率较2011年下降,持续质量改进小组护理人员自评综合能力提高(P<0.01)。结论运用FOCUS-PDCA程序,有效降低住院患者非计划性拔管发生率,对护理人员综合能力提升具有促进作用,有利于临床护理质量持续优化。Objective To explore the application effects of FOCUS-PDCA program to reduce the rate of unplanned extubation （UEX） in hospitalized patients.Methods The reasons were analyzed by 9 steps of FOCUS-PDCA,including find,organize,clarify,understand,select,plan,do,check and act.Improvement measures were formulated.Results The rate of UEX was lower in 2012 than that in 2011.Self evaluation comprehensive ability of nursing staff were improved （P ＜ 0.01）.Condusion The application of FOCUS-PDCA program can reduce the rate of UEX in hospitalized patients effectively,and has stimulative effect to enhance the comprehensive ability of nursing staff,and help to optimize the quality of clinical nursing.
