摘  要:目的探讨不同剂量苯磺酸左旋氨氯地平对高血压患者大动脉弹性和颈动脉粥样斑块的影响。方法根据原发性高血压患者对苯磺酸左旋氨氯地平的反应不同,分为2组,A组为低剂量组(≤2.5 mg/d,n=38),B组高剂量组(≥5 mg/d,n=24),另设一C组作为正常对照(n=30),A、B组均以≤140/90 mmHg为目标血压,连续应用1年。用高分辨率超声检测肱动脉内皮依赖性舒张功能、颈动脉内中膜厚度、颈动脉粥样斑块Crouse积分,应用血管回声跟踪技术(ET)检测颈动脉弹性参数。结果高血压组与健康组比较,肱动脉内皮依赖性舒张功能减退,颈动脉内中膜增厚、颈动脉粥样斑块Crouse积分增加,脉搏波传导速度PWVβ、硬度指数β、增大指数AI、压力-应变弹性系数增加,顺应性AC下降(P<0.05)。治疗后,A、B两组在肱动脉内皮依赖性舒张功能、颈动脉粥样斑块Crouse积分、脉搏波传导速度PWVβ、硬度指数、顺应性差异均有显著意义。B组(高剂量组)较A组(低剂量组)在肱动脉内皮依赖性舒张功能、颈动脉粥样斑块Crouse积分、脉搏波传导速度PWVβ值差异显著。结论苯磺酸左旋氨氯地平在降压的基础上,还具有逆转大动脉功能和减缓粥样斑块形成的作用,这种作用具有一定的剂量相关性。Objective To discuss the effects of different doses of benzene sulfonic acid left-hand amlodipine on artery elasticity and carotid artery atheromatous plaque in hypertension patients.Methods The primary hypertension patients were divided into 2 groups according to their react to benzene sulfonic acid left-hand amlodipine.Group A is low-dose group(≤2.5 mg/d,n=38),group B is high dose group(≥5 mg/d,n=24),group C was set as normal control.The target blood pressure was ≤140/90 mmHg,continuous application for 1 year.The high resolution ultrasound was used to detect brachial artery endothelial dependence diastolic function,carotid intima-media thickness,carotid artery atheromatous plaque Crouse integral,and vascular echo tracking technology(ET) for the detection of carotid elastic parameters.Results Compared with the normal controls,patients in the high blood pressure group had the following changes: brachial artery endothelium-dependent relaxed,carotid intima-media thickened,carotid artery atheromatous plaque Crouse integral increased,the pulse wave conduction velocity,hardness index and increase index,stress-strain elastic coefficient increased,compliance declined.After treatment,patients of group A and B had significant differences in the brachial artery endothelial dependence diastolic function,carotid artery atheromatous plaque Crouse integral,pulse wave conduction velocity,hardness index.There were significant differences in brachial artery endothelial dependence diastolic function,carotid artery atheromatous plaque Crouse integral,pulse wave conduction velocity PWV beta value between patients in group A and B.Conclusion Benzene sulfonic acid left-hand amlodipine can reverse aorta function and slow atheromatous plaque formation,and is correlation with dose,except for decompression.
