听骨链重建中自体乳突皮质骨搭桥术的疗效观察

摘　　要：目的探讨自体乳突皮质骨搭桥术在听骨链重建中的临床应用效果。方法回顾性分析31例自体乳突皮质骨搭桥听骨链重建患者,将自体乳突皮质骨塑成柱形后桥接于锤骨柄和活动的镫骨头或镫骨底板之间,观察术后疗效。结果随访时间半年至2年,术后均获干耳,无面瘫发生,无肉芽、胆脂瘤等复发,无一例自体乳突皮质骨脱出,气导听力提高≥15 dBHL 17例(61.29%),鼓膜再穿孔3例。结论用自体乳突皮质骨塑成柱形后桥接于锤骨柄和活动的镫骨头或镫骨底板之间重建听骨链,费用低廉,效果较稳定,并发症少,听力恢复较理想,是重建锤-镫连接的有效方式之一。Objective To discuss the clinical effect of utilizing self- mastoid cortex bypass surgery in the reconstruction of ossicular chain. Methods Retrospective analysis was performed in 31 patients received reconstruction of ossicular chain, using self - mastoid cortex connecting malleus handle to mobile stapes top or stapes footplate, and the long - term effect was observed. Results In the follow - up period of six months to two years, all the patients had dry ears, and had no facial paralysis, granulation, and cholesteatoma recurrence. There was no self - mastoid cortex prolapse, either. Air conduction improvement greater than or equal to 15dBHL were in 17 cases（61.29% ）. The tympanic membrane perforate occurred again in 3 cases. Conclusion In the reconstruction of ossicular chain, the use of the self - mastoid cortex in connecting malleus handle to mobile stapes top or stapes footplate has the advantage of inexpensive cost, stable effect, less complications and better hearing recovery, which is an effective surgical approach to connecting the malleus and stapes.
