0级糖尿病足中药足浴优化护理方案的研究

摘　　要：[目的]通过科学的方法确定中药足浴治疗0级糖尿病足中医护理操作方案。[方法]运用正交设计法,对中药足浴时的4个因素及对应的3个水平按L9(34)分组设计,制定足浴方案,将45例0级糖尿病足病人首先按照病人适宜温度(36℃、38℃、40℃)分组,然后随机分配进入各试验方案,分别采用9种不同的方案进行足浴。干预3个月后,观察干预前后踝肱指数(ABI)、趾肱指数(TBI)、震动感觉阈值(VPT)的变化。[结果]方差分析结果显示,对于ABI、TBI、VPT的改善方面,药液深度与浸泡时间为主要因素(P<0.05),药液温度与时间安排为次要因素(P>0.05);药液深度达踝上20cm、浸泡30min,药液温度与时间的选择应根据病人的具体情况选择合适的水平。[结论]对0级糖尿病足优化的中药足浴护理方案为:足浴时药液深度达足踝上20cm,浸泡时间为30min,足浴温度根据病人的不同情况选择36℃～40℃,时间安排根据病人具体情况选择19:00～21:00。Objective：Through the scientific method to determine the tradi-tional medical care operation program in treatment of foot bath with tradi-tional Chinese medicine for patients with 0level of diabetic foot.Methods：The use of orthogonal design method,four factors in traditional Chinesemedicine foot bath and corresponding three levels were grouped and de-signed according to L9（34）,then to make foot bath programs.45cases ofpatients with 0level of diabetic foot were first divided into groups based nappropriate temperature（36℃,38℃,40℃）,and then randomly assignedinto each testing program,nine different programs were used for foot bath.After 3months of intervention,ankle-brachial index（ABI）,toe-brachialindex（TBI）,vibration perception threshold（VPT）changes were observedbefore and after intervention.Results：Analysis of variance showed that forimprovement of ABI,TBI and VPT,liquid depth and immersion time werethe main factors（P0.05）,liquid temperature and time arrangement weresecondary factors（P0.05）;drug liquid depth reached to ankle 20cm andsoaked for 30min,the level of the liquid temperature and timing of the se-lection should be based on the specific circumstances of the patient tochoose the right level.Conclusion：The optimized care programs of tradi-tional Chinese medicine foot bath for 0level of diabetic foot：the liquiddepth reached to ankle 20cm when foot bath,soaking for 30min,the tem-perature according to the patient＇s different conditions to choose 36℃～40℃,time arrangement was selected at 19：00～21：00according to the specif-ic circumstances of the patients.
