摘 要:目的研究血清降钙素原(PCT)联合血培养检测对菌血症的诊断价值。方法对137例发热疑似菌血症患者首诊时检测PCT,并同时进行血培养,分析比较确诊患者的首检PCT阳性率、血培养首日和5d阳性率,以及首检PCT阳性患者的菌血症确诊率。结果在确诊的107例菌血症患者中首次检测PCT阳性为91例,阳性率为85.0%;血培养首日结果为阳性的27例,阳性率为25.2%,5 d后血培养阳性总计43例,血培养阳性率为40.2%。137例疑似患者中,首次检测PCT阳性为99例,其中91例确诊为菌血症,确诊率为91.9%。结论PCT检测对菌血症有较好的敏感性,可作为早期诊断菌血症的参考依据。PCT detection and blood culture were performed in 137 fever patients suspected of bacteremia. The positive rates of the PCT at the first test and the blood culture on the first day and the fifth day were analyzed and compared. The rate of the diagnosed bacteremia in those with positive PCT was also analyzed. Results The serum ]?CT was positive in 91 of the 107 patients with diag- nosed bacteremia, accounting for 85%. The blood cuhure was positive in 27 cases accounting for 25.2% on the first day, and in 43 cases accounting for 40.2% on the fifth day. In the 137 patients suspected of bacteremia, 99 were PCT-positive at the first test, accounting for 91.9%. Conclusion As an approach to early diagnosis, PCT detection is of good sensitivity to bacteremia.
