血浆脑利钠肽在川崎病患儿诊断中的临床价值分析

摘　　要：目的:探讨脑利钠肽(BNP)在川崎病(KD)患儿血浆中的变化意义及其在KD临床诊断中的指导价值。方法:选取我院门诊、健康体检和住院病人KD及IKD患儿各10例(Ⅰ组、Ⅱ组),并随机选取10例上呼吸道感染的患儿作为对照组(Ⅲ组)。3组均于入院当日或接受相关治疗12～24h后采用酶联免疫吸附法(ELISA)测定。结果:Ⅰ组KD、Ⅱ组IKD患儿急性期各项实验室检查指标血浆BNP、WBC均明显高于恢复期及对照组,差异有统计学意义(P<0.05);而血浆BNP与ESR的变化无显著相关性(P>0.05),其中BNP水平与CRP及cTn I水平成正相关;常规超声心动图检测Ⅰ组、Ⅱ组急性期LVEF在恢复期有所升高(P<0.05),且BNP水平与LVEF呈负相关;而血浆BNP与CI、LVDd及E/A无显著相关性(P>0.05)。结论:血浆BNP在KD的临床诊断中具有一定的指导价值。Objective： To investigate the brain natriuretic peptide（BNP） in plasma of children with Kawasaki disease（KD） in the change and its significance in KD clinical diagnosis in guiding value.Method： Select the outpatient department of our hospital,health examination and inpatient children with KD and IKD in 10 cases（group Ⅰ,Ⅱ group）,and 10 randomly selected cases of upper respiratory tract infection in children as compared to the control group（group Ⅲ）.The 3 groups from the day of admission or treatment is 12～24h by enzyme linked immunosorbent assay（ELISA） determination.Results： In group Ⅰ KD,Ⅱ IKD in acute stage of the laboratory testing of plasma BNP,WBC were significantly higher than those in recovery period and the control group,the difference was statistically significant（P＆lt; 0.05）;and the plasma levels of BNP and ESR changes had no significant correlation（P＆gt;0.05）,where BNP and CRP and cTn I levels are related;conventional ultrasound heartbeat detection in group Ⅰ,Ⅱ group of acute LVEF during the recovery period was increased（P＆lt; 0.05）,and BNP levels were negatively correlated with LVEF;BNP and CI,and plasma LVDd and E/A has no significant correlation（P＆gt;0.05）.Conclusion： Plasma BNP in KD in clinical diagnosis has the certain instruction value.
