女性下生殖道自窥检查法临床意义的研究

摘　　要：目的:本研究通过自主研发的阴道自窥器应用到女性人群,旨在使妇女能够自主地观察到自身下生殖道的宫颈管及黏液,以及阴道黏膜、白带性状、宫颈上皮等内容,以期为生育调节(避孕与助孕)、生殖道感染防治、宫颈癌防治等多个领域提供一种新的有效途径。方法:选取100名生育期妇女为受试对象,分别对其进行方法学指导,使其分别应用Billings法和阴道自窥检查法观察宫颈黏液变化3个周期,并同时行阴道超声检查监测卵泡发育,以此为标准对两组所测得的易受孕期的开始日、极期日及其长度进行比较。结果:研究产生了211个周期的有效数据,其中200个(94.79%)周期有"阴道自窥检查"极期,165个(78.20%)周期有"Billings法"极期,所测得的易受孕期的开始日、极期日及其长度,"Billings法"组分别为9.9d、16.3d和10.9d,"阴道自窥检查法"组分别为11.8d,16.5d和6.7d。结论:阴道自窥检查法比Billings法更能精确地确定易受孕期的开始日、极期日及易受孕期的长度,更能有效的指导人们进行助孕或避孕。Objectives： The purpose of this study was to provide women a vaginal speculum which could be used by them- selves to examine their cervical mucus, cervical epithelial tissue and vaginal mucosa etc, hoping to establish an effective way to help women and their couples to achieve or avoid pregnancy, as well as to prevent and cure vaginal infections and cercal cancer etc. Methods： One - hundred women in reproductive age were selected and were taught how to observe their cervical mucus and then made self - visual vaginal examination for 3 cycles, at the same time, we monitored the ovulation by transvaginal ultrasonog- raphy as standards to compare the beginning day, the peak day and length of the fertile phase in one menstrual cycle as deter- mined by two methods. Results ： This study generated 211 cycles of valid data, of which 200 cycles （ 94.79% ） had a self- visu- al vaginal examination peek and 165 （78.2%） had a Billings group peek. The beginning day, the average first day of peak fertil- ity and the mean length of the fertile phase determined by self- visual vaginal examination group was day 11.8, day 16.5 and 6. 7 days respectively. Day 9.9, day 16.3 and 10.9 days were determined by using Billings group. Conclusions： The self- visual vaginal examination method is more accurate than the Billings method in determining the beginning day, peak day and length of the fertile phase in one menstrual cycle to help women and their couples to achieve or avoid pregnancy.
