Effects of Acarbose on Compliance of Aorta and Carotid Atherosclerotic Plaque in Hypertensive Patients Complicated with Impaired Glucose Tolerance

摘  要:目的探讨阿卡波糖对高血压并糖耐量减低(impaired glucose tolerance,IGT)患者动脉顺应性和颈动脉粥样硬化的影响。方法选择高血压并IGT患者63例,随机分为常规治疗组(A组)31例,阿卡波糖加常规治疗组(B组)32例;观察治疗前后血压、血糖、餐后2h血糖(2-hour postprandial blood glucose,2hPG)一氧化氮(nitric oxide,NO)、内皮素(endothelin,ET)、肱-踝脉搏波速度(brachil-ankle pulse wave velocity,baPWV)、颈动脉内膜-中膜的厚度(carotid intima-medial thickness,IMT)及颈动脉斑块积分(accumulative score of carotid plaque)的变化。结果治疗6个月后,2组的ET水平及baPWV较治疗前显著下降(P<0.01),NO水平显著上升(P<0.01),B组2hPG,IMT及颈动脉斑块积分较治疗前显著下降(P<0.01);B组的2hPG、ET水平、baPWV、IMT和颈动脉斑块积分明显低于A组(P<0.05),NO水平高于A组(P<0.05);A组IMT和颈动脉斑块积分治疗前后无显著性变化(P>0.05)。结论阿卡波糖可以降低高血压并IGT患者的baPWV,IMT和颈动脉板块积分,改善大动脉的弹性及顺应性。Objective To study effects of acarbose on compliance of aorta and carotid atherosclerotic plaque in hypertensive patients complicated with impaired glucose tolerance (IGT). Methods A total of 63 hypertension patients with IGT were selected and randomized divided into two groups. One (Group A,n=31) received conventional treatment, the other (group B,n=32) received acarbose based on the conventional treatment. The levels of blood glucose,2- hour postprandial blood glucose (2hPG) ,nitric oxide(NO),endothelin(ET)and blood pressure,brachil-ankle pulse wave veloccity(baPWV),carotidintimamedial thickness (IMT) and accumulative score of carotid plaque were measured respectively before and after six months treatment. Results At 6 months after treated with conventional treatment and acarbose combined conventional treatment respectively, levels of ET and baPWV were significantly decreased than that before treatment in group A and group B (all P〈0.01), while level of NO obviously increases (P〈0.01), and 2hPG, IMT and accumulative score of carotid plaque was obviously lowered compared with that before treatment in group B (P〈0.01). After acarbose treatment, levels of 2hPG, ET, baPWV, IMT and accumulative score of carotid plaque in group B were significantly lower those that in group A (P〈0.05), while level of NO was higher than that in group B (P〈0.05).There were no significantly changes of IMT and accumulative score of carotid plaque before and after treatment in group A (P〉0.05). Conclusions Acarbose can decrease baPWV, IMT and accumulative score of carotid plaque of hypertensive patients complicated with IGT, improve elasticity and compliance of aorta, delay and reverse development of arteriosclerosis in IGT patients at early stage.
